Pressure sore definition

Pressure sore means a breakdown in skin integrity caused by immobility and prolonged pressure. The 4 stages of pressure sores can be identified as follows: stage 1 – a nonblanching macule that may appear red or violet; stage 2 – a skin breakdown as far as the dermis; stage 3 – a skin breakdown into the subcutaneous tissue; stage 4 – penetrate bone, muscle or the joint.
Pressure sore means ischemic ulceration and/or necrosis of tissues overlying a bony prominence that has been subjected to pressure, friction or shear. SOM, App. PP, Guidance To Surveyors, Tag F314. Pressure sore and pressure ulcer are synonymous as used in this decision.
Pressure sore means ischemic ulceration or necrosis of tissues overlying a bony prominence that has been sub jected to pressure, friction or shear. The staging system that follows is one method of describing the extent of tis sue damage in the pressure sore. Pressure sores cannot be adequately staged when covered with eschar or necrot ic tissue.
