Policy form definition

Policy form means the form on which the policy is delivered or issued for delivery by the issuer.

Policy form means any policy, contract, certificate, rider, endorsement, evidence of coverage of any amendments thereto that are required by law to be filed with the commissioner for approval prior to their sale or issuance for sale in this state.
