Points Record means a record in relation to a card account for the purpose of determining when you become entitled to a claim a reward.

Examples of Points Record in a sentence

  • Reward Points will ordinarily expire within 36 months of 31 December of the year in which the Reward Point was added to a Points Record.

  • Please note that under clause 10, if ANZ or the Account Holder closes the Card account and ANZ does not receive the Account Holder’s Qantas Frequent Flyer membership number within 60 days after the date of closure, the Points in the Points Record in respect of the Card as at that date will be cancelled and may not be credited to a Qantas Frequent Flyer program account.

  • Rewards may only be redeemed, in respect of Reward Points allocated to your Reward Points Record, in accordance with the Rewards Catalogue current at the time you claim a Reward.

  • If You fail to comply with these terms and conditions or You or Your Additional Cardholder give incorrect information to ANZ, ANZ may cancel Points in your Points Record.

  • All Reward Points recorded in your Reward Points Record will not be redeemable, and will be cancelled and forfeited as at the date of cancellation.

  • Any person other than the Member is eligible to redeem Reward Points accrued on the Member’s Reward Points Record on behalf of the Member if the Member has signed an authorisation acceptable to Diners Club.

  • Unless we advise otherwise, Rewards cannot be returned for Reward Points to a Points Record, cashed in or exchanged.

  • Points in a Points Record at the time of death of the Account Holder will be cancelled and may not be claimed by any other person.

  • You will have no recourse against any Bonus Partner in respect of Points recorded in Your Points Record.

  • Any Reward Points accrued on the Member’s Reward Points Record will be forfeited and void from the date of termination.

  • s Record Subrecipient’s records shall be sufficient to:

  • Project Record means the separate set of Drawings and Specifications as further set forth in paragraph 4.02A.

A personnel record shall include a record in the possession of a person, corporation, partnership, or other association who has a contractual agreement with the employer to keep or supply a personnel record as provided in this subdivision. A personnel record shall not include:

  • Pupil Records Means both of the following: (1) Any information that directly relates to a pupil that is maintained by LEA and (2) any information acquired directly from the pupil through the use of instructional software or applications assigned to the pupil by a teacher or other local educational LEA employee.

  • Clinical record means a legible electronic or hard-copy history that documents the criteria established for medical records as set forth in rule 441—79.3(249A). A claim form or billing statement does not constitute a clinical record.

  • Medical record means any document or combination of documents, except births, deaths, and the fact of admission to or discharge from a hospital, that pertains to the medical history, diagnosis, prognosis, or medical condition of a patient and that is generated and maintained in the process of medical treatment.

  • Email address of record means the designated email

  • Architect of Record means the architect licensed by the State who has the contract responsibility for the Project, who designs and prepares the construction documents from which the building is constructed, and who signs the required documents.

  • Patient Record means a written account of all services provided to a patient by the home health agency, as well as other pertinent information necessary to provide care.

A research record includes, but is not limited to, grant or contract applications, whether funded or unfunded; grant or contract progress and other reports; laboratory notebooks; notes; correspondence; videos; photographs; X-ray film; slides; biological materials; computer files and printouts; manuscripts and publications; equipment use logs; laboratory procurement records; animal facility records; human and animal subject protocols; consent forms; medical charts; and patient research files.

  • Vital record means that term as defined in section 2805 of the public health code, 1978 PA 368, MCL 333.2805.

  • Case record means the file of personally identifiable information, whether written or electronic in form, on an individual that is collected to carry out the purposes of the division as defined in the Act and the Social Security Act. This information remains a part of the case record and is subject to these rules even when temporarily physically removed, either in whole or in part, from the file folder in which it is normally kept.

  • Criminal history record information means that term as defined in section 1a of 1925 PA 289, MCL 28.241a.

  • address on record means the address of the recipient as available in the records of the supplier;

  • System of records means a group of any records under the control of any agency from which information is retrieved by the name of the individual or by some identifying number, symbol, or other identifying particular assigned to the individual.

  • Participant’s records means the records prepared and maintained by the Contractor (in whatever form or storage media) concerning an individual Participant.

  • Inspection Period means the period beginning on the Effective Date and expiring at 5:00 p.m. eastern time on the forty-fifth day after the Effective Date.
