Plan Specifications definition

Plan Specifications means the section of this plan which shows, among other things, the Eligibility Requirements, Eligibility Waiting Period, Elimination Period, Amount of Benefits, Minimum Benefit, and Maximum Benefit Period.
Plan Specifications means the RHFP® Plan Specifications document attached hereto and incorporated herein by reference, by which the Employer/Sponsoring Entity establishes or by which an Adopting Employer/Sponsoring Entity adopts a welfare benefit plan pursuant to the terms of this Plan for the benefit of its Eligible Employees.
Plan Specifications means in relation to each Member the details of the following matters determined in accordance with Clause 2.1:

Examples of Plan Specifications in a sentence

  • Subscription: Customer agrees to subscribe to the Wireless Plan Specifications above and to pay OptConnect the monthly Wireless Service Charge as stated above.

  • Customer enters into this Agreement with OptConnect and agrees to rent the equipment described above, to subscribe to the Wireless Plan Specifications above and to be bound by and subject to the following terms and conditions: Equipment: OptConnect may fill Customer's order with new or refurbished equipment as described above.

  • At United’s request and in accordance with applicable United policies and procedures, Administrator shall process initial benefit and coverage determinations and exception requests and shall provide support to United in connection with appeals and grievances that are processed under the Benefit Plans in accordance with the Pharmacy Plan Specifications and applicable Laws and Regulations.

  • United shall provide Administrator with the technical assistance and information Administrator reasonably needs to perform the Services under this Agreement, including, without limitation, information regarding Members, the Benefit Plans, and the Pharmacy Plan Specifications.

  • United shall provide Administrator with the Pharmacy Plan Specifications no later than ninety (90) days prior to the Commencement Date of this Agreement and the Specifications will be implemented by Administrator on or before the 90th day.

  • In addition to the specific Services addressed in this Exhibit C, Administrator shall perform any and all duties incident thereto which may be necessary or appropriate to fully carry out the Services and the administration of the Benefit Plans in accordance with the Pharmacy Plan Specifications, this Exhibit C and the Agreement.

  • Administrator will dispense Covered Prescription Services to Members in accordance with the terms and conditions of this Agreement, including any exhibits, schedules and addenda attached hereto, and the Pharmacy Plan Specifications.

More Definitions of Plan Specifications

Plan Specifications means a description of CLIENT’s Plan related to Prescription Drug benefits and coverage limitations, including the framework of policies, interpretations, rules, practices and procedures applicable to such benefits, required by CLIENT and set forth in a mutually agreed upon document.

Plan Specifications means the information made available by PBF to Participating Pharmacy describing, among other things, the applicable processes and Covered Services under a particular Sponsor’s health benefit plan and all related amendments, provided by PBF to Participating Pharmacy in accordance with this Agreement. This information may include the amounts to be reimbursed to Participating Pharmacy for Covered Services; excluded items; Cost Sharing Amounts; benefit maximums; coverage or payment limitations; and other items, all subject to the actual requirements of that Sponsor’s health benefit plan.

Related to Plan Specifications

  • Contract Specifications means the principal trading terms in CFD (for example Spread, Swaps, Lot Size, Initial Margin, Necessary Margin, Hedged Margin, the minimum level for placing Stop Loss, Take Profit and Limit Orders, financing charges, charges etc) for each type of CFD as determined by the Company from time to time.

  • Standard Specifications means a compilation in book form of specifica- tions approved for general application and repetitive use;
