Plan Design means Program drug coverage, days’ supply limitation, Copayment/Coinsurance, Formulary (including Formulary drug selection) and other Program specifications applicable to the Program designated by SPONSOR as set forth in this Agreement or otherwise documented between the parties.
Plan Design means the group dental insurance benefits, as set forth in Attachment C: Plan Design. “Plan Year” is based on the calendar year from January 1 to December 31.

More Definitions of Plan Design

Plan Design means the covered prescription benefits, participant contributions and limitations adopted by the Sponsor as outlined in Schedule A of this Agreement.
Plan Design means the Pharmacy Benefit Plan Design Form as agreed upon by both parties which includes the setup of the Group Health Plan.
Plan Design means the Covered Services, exclusions, co-payment, formulary (including formulary drug selection cost indications, and formulary compliance programs), and other specifications applicable to each Group.

Related to Plan Design

  • Plan Proponents means the Debtors and the Committee.

  • EHC plan means an Education, Health and Care plan made under sections 37(2) of the Children and Families Act 2014.

  • Retirement Committee means a committee consisting of the Company’s Vice President of Human Resources, the Director of HR Operations and the Compensation & Benefits Manager.

  • Appeal Committee means the appeal committee established by the Council in terms of section 12(3)(a);

  • Plan Committee means a committee of two or more directors appointed by the Board to administer the Plan.

  • Flexi Plan means any individual indemnity hospital insurance plan under the VHIS framework with enhancement(s) to any or all of the protections or terms and benefits that the Standard Plan provides to the Policy Holder and the Insured Person, subject to certification by the Government. Such plan shall not contain terms and benefits which are less favourable than those in the Standard Plan, save for the exception as may be approved by the Government from time to time.

  • Deferral Plan means the UGI Corporation 2009 Deferral Plan.

  • Claim for Benefits means a request for a Plan benefit or benefits made by a Member in accordance with the Plan’s Appeals Procedures, including any Pre-Service Claims (requests for Prior Authorization) and Post-Service Claims (requests for benefit payment).

  • Plan hearing means the public hearing on the draft Project Area Plan required under Subsection 17C-1-102 (41) and 17C-5-104(3)(e).

  • Benefits Committee means the Employee Benefits Committee of Textron.

  • BCDR Plan means the plan consisting of general business continuity and disaster recovery principles, the Business Continuity Plan and Disaster Recovery Plan as further described in paragraph 1.2 of Schedule 2- 14.

  • Summary Plan Description means this plan document and summary plan description.

  • Notice Plan means and refers to the plan to disseminate Notice of the Settlement Agreement to the Settlement Class that comports with due process.

  • Plan Agreement means a written agreement, as may be amended from time to time, which is entered into by and between an Employer and a Participant. Each Plan Agreement executed by a Participant and the Participant’s Employer shall provide for the entire benefit to which such Participant is entitled under the Plan; should there be more than one Plan Agreement, the Plan Agreement bearing the latest date of acceptance by the Employer shall supersede all previous Plan Agreements in their entirety and shall govern such entitlement. The terms of any Plan Agreement may be different for any Participant, and any Plan Agreement may provide additional benefits not set forth in the Plan or limit the benefits otherwise provided under the Plan; provided, however, that any such additional benefits or benefit limitations must be agreed to by both the Employer and the Participant.

  • Plan Sponsor has the meaning given in ERISA Section 3(16)(B).
