Plan Data definition

Plan Data means reasonably accessible data in the possession, custody, and control of Defendants that is necessary to implement the Plan of Allocation, including but not limited to, for each Class Member (including Former Participants and Beneficiaries), their name, most current address, email address, Social Security numbers or other individual identifiers as needed to effectuate this Agreement, and individual Plan account allocations during the Class Period to the funds included in the Plan of Allocation on a quarterly basis.
Plan Data means the PBM's standard Claims data in National Council for Prescription Drug Programs ("NCPDP") or X12 815 formats as well as all pricing information utilized by PBM in the adjudication of pricing or Guarantee calculations for Paid Claims including but not limited to MAC Lists, Specialty Lists, etc. Plan Data shall not include Participating Pharmacy agreements nor PBM agreements with pharmaceutical manufacturers including without limitation agreements for Manufacturer Derived Revenue.
Plan Data means non-public Plan level information that is provided to Empower in connection with receipt of the Services. Plan Data excludes data that is de-identified and aggregated for benchmarking and research purposes.

More Definitions of Plan Data

Plan Data means (i) all records, data and information provided to the Recordkeeper by the Sponsor or the Plan's previous recordkeeper and (ii) all data and information created by the Recordkeeper by processing the records, data and information provided by the Sponsor or its agent or as a result of providing the recordkeeping services.
Plan Data shall have the meaning set forth in Schedule 12.4.
Plan Data means all records and data received, generated, used, stored or acquired by CONTRACTING PROVIDER pursuant to this Agreement or in furtherance of CONTRACTING PROVIDER's obligations under this Agreement, but shall not include any information, documentation or materials which were CONTRACTING PROVIDER's property or work product prior to the Effective Date. All Plan Data is and shall remain the exclusive property of Anthem BC&BS and shall be returned to Anthem BC&BS, at its request and at no additional charge, promptly upon termination of CONTRACTING PROVIDER's duties under this Agreement in the format designated by Anthem BC&BS. CONTRACTING PROVIDER hereby waives any interest, title, lien or other right to any Plan Data. Plan Data will be returned to Anthem BC&BS in good order.

  • Summary data means statistical records and reports derived from data on individuals but in which individuals are not identified and from which neither their identities nor any other characteristic that could uniquely identify the individual is ascertainable.

  • Encounter Data Any Contractor accessing payments for services through the Global Commitment to Health Waiver and Vermont Medicaid programs must provide encounter data to the Agency of Human Services and/or its departments and ensure that it can be linked to enrollee eligibility files maintained by the State.

  • Teacher Data is defined as at least the following: Social security number. Date of birth. Personal street address. Personal email address. Personal telephone number Performance evaluations.   • Data Source Annual Fiscal Audit Report, Terms of Debt, Other Formal Notifications Received by School. Default Rubric Exceeds Standard The school has met standard for at least 3 consecutive years. Meets Standard The school is not in default of any financial obligations, and did not experience any instances of default during the fiscal year. Financial obligations include, but are not limited to, making payments to vendors and utility services on time, complying with all loan covenants, filing any reports required for maintenance of grants or philanthropic funds, meeting all tax obligations, and operating without financial judgements or property liens. Approaches Standard The school experienced one or more instances of minor default during the fiscal year (such as making late payments); however, the school is not currently in default of any financial obligations. Does Not Meet Standard School is currently in default of financial obligations.

  • Disenrollment means either voluntary or involuntary termination of a participant from the Independent Choices Program.

  • 800 service means a telecommunications service that:

  • Pseudonymous data means personal data that cannot be attributed to a specific natural person

  • Computer database or “database” means a collection of recorded information in a form capable of, and for the purpose of, being stored in, processed, and operated on by a computer. The term does not include computer software.

  • GLO Data means any data or information owned by the GLO, including PII or SPI as defined below, that Provider creates, obtains, accesses (via records, systems, or otherwise), receives (from the GLO or on behalf of the GLO), or uses in the performance of the Contract or any documents related thereto.

  • 900 service means an inbound toll telecommunications service purchased by a subscriber that allows the subscriber's customers to call in to the subscriber's prerecorded announcement or live service, and which is typically marketed under the name "900 service" and any subsequent numbers designated by the federal communications commission. "900 service" does not include the charge for collection services provided by the seller of the telecommunications service to the subscriber, or services or products sold by the subscriber to the subscriber's customer.

  • Contractor information system means an information system belonging to, or operated by or for, the Contractor.

  • Anonymous Data means Personal Data that has been processed in such a manner that it can no longer be attributed to an identified or identifiable natural person.

  • Data means recorded information, regardless of form, the media on which it is recorded, or the method of recording.

  • ADHS means the Arizona Department of Health Services.
