Personal Health Record definition

Personal Health Record or “PHR” shall mean an electronic record of health- related information of an Individual which is managed by the Individual or his or her authorized proxy, directly or through a third party application. and which conforms to standards approved by CRISP in the Policies and Procedures.

Personal Health Record means an electronic record of PHR identifiable health information on an individual that has the technical capacity to draw information from multiple sources and that is managed, shared, and controlled by or primarily for the individual.
