Payment Form definition

Payment Form means the manner of payment as specified in Section 2.5.
Payment Form means the Elected Payment Form or, if no such form is elected by a Participant, the Default Payment Form.
Payment Form means the document or documents, voice response or electronic media prescribed by the Plan Administrator pursuant to which a Participant shall elect the time and form for distribution of his or her Account balance, as provided under Article 7.

  • Each invoice submitted to City for payment shall include the information required in the HRC Progress Payment Form and the HRC Payment Affidavit.

More Definitions of Payment Form

Payment Form means the Elected Payment Form or, if no such form is elected or is permitted to be elected by the Participant, the Default Payment Form, in each case for the payment of a Plan Benefit.
Payment Form means the document, documents, or electronic media prescribed by the Plan Administrator pursuant to which a Participant shall elect the time of distribution of his Account balance, and, with respect to the Participant’s initial year of participation except as otherwise set forth in Section 8.1(b)(ii), the form of payment of his Account balance, as provided under Article 8.
Payment Form means the document or documents, voice response or electronic media prescribed by the Committee pursuant to which a Participant shall elect the time and form for distribution of his Account, as provided under Article 8.
Payment Form means, with respect to any Participant, the form of benefit payment elected by the Participant in accordance with the election provisions of Section 2.2. The available Payment Forms are lump sum payment and annual installments over a period of between two and ten years, as elected on a Participant’s election form. If no Payment Form has been elected, the Payment Form shall be a lump sum payment. In addition, Participants may elect to have benefit payments accelerated and paid in a lump sum payment upon Participant’s Disability.
Payment Form means with respect to any Participant the form of payment of the Aggregate Account Balance elected by the Participant in the Participation Agreement most recently signed by the Participant and delivered to the Plan. If no form of payment is made available for election or no other form of payment is so elected or if any form of payment elected is not valid, the Payment Form shall be lump sum. Any Payment Form other than lump sum to be available shall be solely at the discretion of the Administrator with the approval of the Employer.
Payment Form means one of the following forms of payment on Retirement elected in a Participant’s Election: (i) a lump sum payment or (ii) annual installments payments (not to exceed fifteen (15) years).
