Patient Medical Data definition

Patient Medical Data means the following Know-How, data and information [***];
Patient Medical Data means your identifiable personal and medical data and any information you, your legal guardian, or someone acting on your behalf enter into the Service pursuant to the terms and conditions provided herein and which will be shared with healthcare providers. “PHI” means “Protected Health Information” and includes Patient Medical Data and most of the other information that will be provided.

  • Clinical Data means the information concerning the safety or performance that is generated from the use of a device and that are sourced from the following:

  • Patient Data means any electronic data, information or material about a Patient entered into the Software.

Police officers, firefighters, funeral home employees and other persons serving in a dual capacity one of which meets the definition of “emergency medical services provider” are “emergency medical services providers” within the meaning of this chapter.

  • Medical Specialist means any medical practitioner who is vocationally registered by the Medical Council under the Health Practitioners Competence Assurance Act 2003 in one of the approved branches of medicine and who is employed in either that branch of medicine or in a similar capacity with minimal oversight.

  • Emergency medical care provider means an individual who has been trained to provide emergency and nonemergency medical care at the first responder, EMT-basic, EMT-intermediate, EMT-paramedic, paramedic specialist or other certification levels recognized by the department before 1984 and who has been issued a certificate by the department.

  • Information technology services means services designed to do any of the following:

  • Medical provider means a medical service provider, a hospital, a medical clinic, or a vendor of medical services.

  • Behavioral health provider means a person licensed under 34 chapter 18.57, 18.57A, 18.71, 18.71A, 18.83, 18.205, 18.225, or 18.79
