Patient Data definition

Patient Data means any electronic data, information or material about a Patient entered into the Software.
Patient Data means health information that is created or received by a health care provider, payer, employer, or other Covered Entity and relates to the past, present, or future physical or mental health condition of an individual or the provision of health care to an individual and that identifies the individual, or the past, present, or future payment for the provision of health care to an individual, or with respect to which there is a reasonable basis to believe the information can be used to identify the individual, including such information that is made available for exchange by a Data Provider or Data Source pursuant to Section 6 (Data ProvidersRights and Obligations).
Patient Data means information provided, or made available for exchange, by a Data Provider through HIO’s System and Services pursuant to Section 7.2 (Provision of Data).

Examples of Patient Data in a sentence

  • SACVALLEY MEDSHARE shall not use Patient Data for any purpose other than that of rendering services under this Agreement, nor sell, assign, lease, dispose of or otherwise exploit Patient Data.

  • Notwithstanding anything to the contrary, SACVALLEY MEDSHARE represents, warrants, and covenants that neither SACVALLEY MEDSHARE nor any of SACVALLEY MEDSHARE’s personnel or contractors, other than ICA, will directly access any Patient Data in the Exchange or Hosted System, except with the written consent of the Participant that provided that Patient Data.

  • All such updates, modifications, and derivatives constitute Participant Patient Data hereunder.

  • If Participant requests at any time, SACVALLEY MEDSHARE shall destroy all copies of Participant Patient Data in SACVALLEY MEDSHARE’s possession or control.

  • Participant agrees to include in its notice of privacy practices under HIPAA that Patient Data will be provided to an HIO.


More Definitions of Patient Data

Patient Data means any Patient Data concerning the health of a patient and including in particular any related data from various data sources (e.g. IT middleware, electronic medical record systems) that forms Patient Data and is automatically or manually uploaded into the Software.
Patient Data means data relating to an individual which are held for medical purposes (within the meaning of section 251 of the National Health Service Act 2006) and from which the individual can be identified.
Patient Data means data Patient data. – Data that includes a patient's age, sex, race, ethnicity, zip code, third-party coverage, principal and other diagnosis, diagnoses, date of admission, procedure and discharge date, principal and other procedures, total charges and components of the total charges, attending physician identification number, and hospital or freestanding ambulatory surgical facility identification number.
Patient Data means information provided by a Data Provider pursuant to Section 7.2 (Provision of Data).
Patient Data means electronic health, demographic and related information provided by a Data Provider to Data Recipients pursuant to Section 6.2 (Provision of Data).
Patient Data means health information that: (a) is created or received by a Healthcare Provider or Health Plan; (b) relates to: (i) past, present or future physical or mental health of a Patient, or (ii) the provision of health care to a Patient; (c) identifies the Patient, or there is a reasonable basis to believe the information can be used to identify the Patient (including Protected Health Information, as that term is defined in HIPAA, and Medical Information, as that term is defined in the CMIA); and (d) is made available to the System by a Data Provider or Data Contributor pursuant to the Agreement or an NP Participant’s participation agreement.
Patient Data means Personal data that is transferred between the Data Controller and iPlato to identify suitable individuals to be contacted via the system. Personal Data may also include patient records that exist within the GP Practice clinical systems, including patient demographic information, patient medical records and all appointment information regarding patients of the GP Practice, although it is acknowledged that in this type of Patient Data is not covered by this Data Processing Agreement but is covered with individual Data Processing Agreements that iPlato has with individual GP Practices.
