Infertility treatment definition

Infertility treatment means procedures consistent with established medical practices in the treatment of infertility by licensed physicians and surgeons including, but not limited to, diagnosis, diagnostic tests, medication, surgery, and in vitro fertilization.
Infertility treatment means any services, supplies or drugs related to the diagnosis or treatment of infertility.
Infertility treatment means services, tests, supplies, devices, or drugs which are intended to promote fertility, achieve a condition of pregnancy, or treat an illness causing an infertility condition when such treatment is done in an attempt to bring about a pregnancy.
