Identifiable Information an individual's last name and the individual's first name or first initial, in combination with and linked to any of the following elements, if the element is not publicly available information and is not encrypted, redacted, or altered in any manner that renders the element unreadable:
a) The individual's Social Security number;
b) The individual's driver's license number or state identification number;
c) The individual's date of birth;
d) The number of the individual's financial account, including a credit or debit card account number, or any security code, access code, or password that would permit access to the individual's financial account;
e) The individual's DNA profile; or
f) The individual's unique biometric data, including fingerprint, voice print, retina or iris image, or any other unique physical characteristic.
Identifiable Information as defined by DHHS means information that is individually identifiable (i.e., the identity of the subject is or may readily be ascertained by the investigator or associated with the information).
Identifiable Information. “PII”) shall mean the following:
a. An individual’s social security number; and/or
b. An individual’s initials, first name, or first initial and last name in combination with any one or more of the following data elements for that individual:
i. A driver’s license or identification card number, passport number, military identification number, or other similar number issued on a government document used to verify identity;
ii. A financial account number or credit or debit card number in combination with any required security code, access code, or password that is necessary to permit access to an individual’s financial account;
iii. Any information regarding an individual’s medical history, mental or physical condition, or medical treatment or diagnosis by a health care professional; or
iv. An individual’s health insurance policy number or subscriber identification number and any unique identifier used by a health insurer to identify the individual.
v. Any other identifier, as referenced in the Department of Health & Human Services “Safe Harbor Standards.”
vi. The term “Personally Identifiable Information” does not include information about an individual that has been made publicly available by a federal, state, or local governmental entity. The term also does not include information that is encrypted, secured, or modified by any other method or technology that removes elements that personally identify an individual or that otherwise renders the information unusable.
c. A user name or e-mail address, in combination with a password or security question and answer that would permit access to an online account.
d. The PII provided pursuant to the Agreement shall be limited to what is necessary for the Business Associate to meet its obligations thereunder.
Examples of Identifiable Information in a sentence
If Signatory is a NHE (but not to the extent that it is acting as an entity entitled to make a Government Benefits Determination under Applicable Law, a Public Health Authority, or a Government Health Care Entity), then it shall comply with the provisions of the HIPAA Privacy Rule listed below with respect to all Individually Identifiable information that Signatory reasonably believes is TI as if such information is Protected Health Information and Signatory is a Covered Entity.
If Signatory is an IAS Provider it must comply with the applicable security requirements set forth in this Common Agreement and the security SOPs for all Individually Identifiable information they hold, regardless of whether such information is TI.
If Signatory is an IAS Provider it is required to encrypt all Individually Identifiable information held by Signatory, both in transit and at rest, regardless of whether such data are TI.
