Hazardous materials incident means any fire, explosion, transportation accident, industrial accident, or any other cause, whatsoever, which results in the actual or threatened or impending leak or spill or other unauthorized release into the environment of a normally and ordinarily contained or otherwise controlled hazardous material; and
