Facility Use Application definition

Facility Use Application. ("Application") means a form approved by the executive director used to apply to reserve Capitol Hill Facilities or Capitol Hill Grounds for an event.

Facility Use Application. - means a form that is to be completed by a prospective user, and approved by a resident agency, to reserve space for activities held within state-owned facilities. It shall require submission of the following information: (i) prospective user’s name, address, and telephone number; (ii) the name of the facility being requested; (iii) the type of activity; (iv) the dates and times of the function; (v) insurance company, name and policy number, unless applicant is seeking a waiver under rule R131-2-4(22); (vi) any other special considerations being requested.
