Exception Request definition

Exception Request means a request made by a Submitting Entity in accordance with Appendix 5C for an Exception.

Exception Request means a process that allows a covered person, authorized representative, or prescribing physician (or other prescriber, as appropriate) to request and gain access to clinically appropriate drugs not otherwise covered by a health benefit plan.

  • The Step Therapy Exception Request will be considered approved if we do not deny the request within 72 hours after receipt of the request.

  • For staff who are related to or acquainted with an offender, the Contractor’s staff member must complete the Offender Contact Exception Request (CAJ-202) as provided by the MDOC and submit it to the MDOC Program Manager or designee.

  • If your prescribing Physician or other Health Care Practitioner reasonably believes that denial of the Step Therapy Exception Request could cause you serious harm or death, the request is considered approved if we do not deny 24 hours after receipt of the request.

  • In the event that a City employee cannot fully comply with one or more element(s) described in this Policy, the employee may request an exception by submitting Security Exception Request.


More Definitions of Exception Request

Exception Request means a request for coverage of a non-formulary drug. If you, your designee, or your prescribing health care provider submits a request for coverage of a non-formulary drug, your insurer must cover the non-formulary drug when it is medically necessary for you to take the drug.

  • Information Request means a request for any Information under the FOI Legislation.

  • Mediation Request shall have the meaning set forth in Section 7.2.

  • Corporation Request and "Corporation Order" mean, respectively, the written request or order signed in the name of the Corporation by its Chairman of the Board of Directors, its Vice Chairman of the Board of Directors, its President or a Vice President, and by its Treasurer, an Assistant Treasurer, its Secretary or an Assistant Secretary, and delivered to the Trustee.

  • Utilisation Request means a notice substantially in the form set out in Schedule 3 (Utilisation Request).

  • Service Authorization Request means a Provider’s or Enrollee’s request to the Contractor for the provision of a service.

  • Registration Request has the meaning set forth in Section 2.01(a).

  • Certification Requirements means those requirements specified or referenced in this Agreement that you must meet in order to use and maintain the Certification Designation and use the Logo in accordance with the terms of this Agreement.

  • Bid Submission Requirement means a task a Prospective Contractor must complete when submitting a bid response. These requirements will be distinguished by using the term “shall” or “must” in the requirement.

  • Loan Request means a letter in substantially the form of Exhibit A hereto executed and delivered by the Borrower to the Administrative Agent and the Group Agents pursuant to Section 2.02(a).

  • Redemption Request has the meaning as set forth in Section 8.1.

  • Conversion Request means a request, substantially in the form of Exhibit B, by the Borrower to convert the interest rate basis for all or portions of outstanding Loans, which shall specify (a) the requested Conversion Date, which shall be not fewer than three Business Days after the date of such Conversion Request, (b) the aggregate amount of such Loans, on and after the Conversion Date, which are to bear interest as ABR Loans or Eurodollar Loans and (c) the term of the Interest Periods therefor, if any.

  • Extension Request has the meaning provided in Section 2.20(a).

  • Generation Interconnection Request means a request by a Generation Interconnection Customer pursuant to Tariff, Part IV, subpart A, to interconnect a generating unit with the Transmission System or to increase the capacity of a generating unit interconnected with the Transmission System in the PJM Region.

  • Qualification Requirements means the qualification requirements as set forth in Section-2, Clause 2.1 of this RFP;

  • Drawdown Request means a notice substantially in the form set out in Schedule 3 (Drawdown Request).

  • Surplus Interconnection Request means a request submitted by a Surplus Interconnection Customer, pursuant to Tariff, Attachment RR, to utilize Surplus Interconnection Service within the Transmission System in the PJM Region. A Surplus Interconnection Request is not a New Service Request.

  • Credit Event Resolution Request Date means, with respect to a notice to ISDA requesting that a Credit Derivatives Determinations Committee be convened to resolve:

  • Notice of Loan means a Notice of Loan in the form of the attached Exhibit D.

  • Borrowing Notice is defined in Section 2.8.

  • Interest Election Request means a request by the Borrower to convert or continue a Borrowing in accordance with Section 2.07.

  • Notice of Revolving Borrowing shall have the meaning as set forth in Section 2.3.

  • Notice of Borrowing has the meaning specified in Section 2.02(a).
