Evaluation Form definition

Evaluation Form. The summative statements of the appraiser after at least two Formal Observations of the appraisee, using the Performance Descriptors.
Evaluation Form means that document which becomes a part of the employee's personnel file. The Employee Final Evaluation Form is attached to and made a part of this Agreement as Appendix B-1. The Educational Staff Association Final Evaluation Form is attached to and made a part of this Agreement as Appendix B-2. The Librarian Final Evaluation Form is attached to and made a part of this Agreement as Appendix B-3. The Counselor Final Evaluation Form is attached to and made a part of this Agreement as Appendix B-4. The School Psychologist Final Evaluation Form is attached to and made a part of this Agreement as Appendix B-5.
Evaluation Form. (the “Form”) means the form created by Procurement Services, as amended from time to time, to be used in documenting and rating Vendor performance management.

Examples of Evaluation Form in a sentence

  • All criteria identified in Attachment F, Proposal Evaluation Form.

  • Consultant or Contractor Selection Committee (“CSC”) members shall use the Proposal Evaluation Form (Exhibit A) for the RFQ to document their review and evaluation of each Proposal in accordance with the criteria listed below, within their respective areas of experience and knowledge.

  • At the conclusion of this meeting, each CSC member shall render his or her final scores for each proposal and submit his or her Proposal Evaluation Form to the Selection Process Administrator.

  • Submittals shall be formatted and tabbed in the exact form and numeric sequence of the Evaluation Form (1 through 5) in Appendix A.

Related to Evaluation Form

  • Annual Information Form means a document filed by the funds with Canadian securities regulators which provides supplementary information about the funds.

  • Evaluation Criteria means the criteria set out under the clause 27 (Evaluation Process) of this Part C, which includes the Qualifying Criteria, Functional Criteria and Price and Preferential Points Assessment.

  • Election Form means the form established from time to time by the Committee that a Participant completes, signs and returns to the Committee to make an election under the Plan.

  • Evaluation Report means a report on the evaluation of tenders prepared under CSO 12

  • Bid Application Form means the form as specified in Annexure II of this E-Auction Process Information Document;

  • Deferral Election Form means the form established from time to time by the Plan Administrator that the Director completes, signs and returns to the Plan Administrator to designate the amount of the Deferrals.

  • Evaluation Team means the team appointed by the City; “Information Meeting” has the meaning set out in section 2.2;

  • Application Form means any application form to be completed by subscribers for Shares as prescribed by the Company from time to time.

  • Evaluation Date shall have the meaning ascribed to such term in Section 3.1(s).

  • Variation Form means the form that will be completed and signed by the Parties to effect a Variation which shall be in the form set out in Framework Schedule 19 (Variation Form);

  • Enrollment Agreement means an agreement between the Company and an employee, in such form as may be established by the Company from time to time, pursuant to which the employee elects to participate in this Plan, or elects changes with respect to such participation as permitted under the Plan.

  • Letter of Transmittal and Election Form means the letter of transmittal and election form(s) to be delivered by Company to the Company Shareholders and Company RSU Holders and providing for the Company Shareholder’s and Company RSU Holders election with respect to the Consideration;

  • Authorization Form means a form specified in s. 440.73.

  • Enrollment Form means an agreement pursuant to which an Eligible Employee may elect to enroll in the Plan, to authorize a new level of payroll deductions, or to stop payroll deductions and withdraw from an Offering Period.

  • Evaluation Period bears the meaning ascribed thereto in Section 7.4(d)(i);

  • Enrollment means the number of students who are enrolled in a school operated by the district on October 1. A student shall be counted as one whether the student is enrolled as a full-time or part-time student. Students enrolled in prekindergarten programs shall not be counted.

  • Subscription Form means the form of subscription annexed hereto as Schedule “A”;

  • Account Opening Application Form means the application form/questionnaire completed by the Client in order to apply for the Company’s Services under this Agreement and a Client Account, via which form/questionnaire the Company will obtain amongst other things information for the Client’s identification and due diligence, his categorization and appropriateness or suitability (as applicable) in accordance with the Applicable Regulations.

  • Evaluation Committee means a body appointed to perform the evaluation of Offerors’ proposals.

  • Beneficiary Designation Form means the form established from time to time by the Plan Administrator that the Executive completes, signs, and returns to the Plan Administrator to designate one or more Beneficiaries.

  • Distribution Election Form means the form established from time to time by the Plan Administrator that the Director completes, signs and returns to the Plan Administrator to designate the time and form of distribution.

  • Tax Certification Forms means any forms or other documentation as may be issued or required by a Tax Authority or by the Bank from time to time to confirm the tax status of the Customer or of a Connected Person.

  • Account Application Form means the Personal Account Application Form for International Personal Banking or Wealth and Investment as applicable;

  • Evaluation means an appraisal of an individual’s professional performance in relation to his or her job description and professional standards and based on, when applicable, the individual’s evaluation rubric.

  • Examination Criteria means the loan classification criteria employed by, or any applicable regulations of, the Assuming Institution’s Chartering Authority at the time such action is taken, as such criteria may be amended from time to time.

  • Effective date of enrollment means the first date when an en- rollee is entitled to receive covered benefits.
