Enrolled Dependent definition

Enrolled Dependent. [or “Enrollee”] – a Dependent who is enrolled for Coverage under this Contract.
Enrolled Dependent means an Enrolled Employee's eligible dependent who is listed on the Enrolled Employee's application, has completed an enrollment form and is enrolled under this Contract. The term "Member" refers to an Enrolled Employee or an Enrolled Dependent. Other terms are defined where they are first used or in the definitions section in the back of the Booklet.
Enrolled Dependent means an Eligible Dependent who is enrolled for Coverage under the health Plan.

Examples of Enrolled Dependent in a sentence

  • When a Dependent enrolls, we refer to that person as an Enrolled Dependent.

  • Coverage for an unmarried Enrolled Dependent child who is incapacitated will not end just because the child has reached a certain age.

  • Please refer to Section 8: Defined Terms for definitions of the terms “Eligible Person,” “Policyholder,” "Dependent" and "Enrolled Dependent."   • Coverage will continue as long as the Enrolled Dependent child is medically certified as incapacitated and dependent unless coverage otherwise ends in accordance with the terms of this Policy.

  • Coverage for an unmarried Enrolled Dependent child who is disabled will not end just because the child has reached a certain age.

  • Our notice to the Subscriber shall be deemed as notice to [the Subscriber's Enrolled Dependents][each Enrolled Dependent] and is sufficient if mailed to the Subscriber's address as it appears in Our records.

  • Whenever a Child under the age of 18 is an Enrolled Dependent under this Contract through a [Subscriber that is a] non-custodial parent, We shall, upon the custodial parent’s written request do all of the following.


More Definitions of Enrolled Dependent

Enrolled Dependent means each of the Dependents whom a Participant has elected to cover under the Plan as his or her Dependent.

GROUP QUALIFICATIONS In order to qualify as an employer and to maintain eligibility for this employer health insurance Contract, the Group must be a bona fide person (including sole proprietors or self-employed individuals), firm, corporation (including Limited Liability Corporations, or LLCs), partnership (including Limited Liability Partnerships (LLPs)), labor union or political subdivision. In order to be eligible, a Group must: accept billing on a consolidated basis and collect any required employee contributions via payroll deductions; be actively engaged in legal business activity; be licensed to conduct business in the state and obtain other business licenses as required by law; employ at least two employees on the first day of the plan year with a normal work week of the number of hours indicated in the Group's application, the majority (75 percent or more) of whom are employed within Our service area and with which a bona fide employer-employee relationship exists; and have a status as a legal entity with authority to contract for health insurance coverage and not be formed primarily for purposes of buyi...
Enrolled Dependent means any Eligible Dependent who has become a Member as a result of the rights and privileges accorded the Enrolled Subscriber under the terms of this Agreement. Such enrollment shall be evidenced by the addition of the dependent’s name on the Enrolled Subscriber's Enrollment Form.

  • Welfare Plan means a “welfare plan” as defined in Section 3(1) of ERISA.

  • Dental plan means any dental insurance policy, including those of nonprofit health service plans, and those of commercial group, blanket, and individual policies, any subscriber contracts issued by Health Maintenance Organizations (HMOs), and any other established programs under which the insured may make a claim. The term Dental Plan includes coverage under a governmental plan, or coverage required to be provided by law. This does not include a State plan under Medicaid (Title XIX, Grants to States for Medical Assistance Programs, of the United States Social Security Act, as amended from time to time.)

  • Qualified Plans has the meaning set forth in Section 5.20.

  • Medicare Advantage plan means a plan of coverage for health benefits under Medicare Part C as defined in 42 U.S.C. 1395w-28(b)(1), and includes:

  • dependants for the purpose of definition, dependants are spouse, dependent children and anyone for whom the employee claims exemption on federal income tax returns;

  • Surviving Spouse means the widow or widower, as the case may be, of a Deceased Participant or a Deceased Beneficiary (as applicable).
