Examples of Eligible Dependent in a sentence
Eligible Dependent – i) the spouse, as determined under applicable state law at the time and location that the marriage was entered into, ii) a Child of an Eligible Employee who satisfies the requirements of Section 5.4 and iii) any such spouse or Child who timely elects Continuation Coverage and for whom the appropriate premium is timely received by Surency.
Enrollee – A person, whether an Eligible Employee or Eligible Dependent, who is i) eligible to be covered by this Agreement, ii) validly enrolled in the Plan, and iii) for whom the appropriate premium is timely received by Surency.
A disabled Child shall continue to be an Eligible Dependent for the duration of the disability, provided: i) his or her status as an Eligible Dependent does not terminate for any other reason, and ii) proof of disability is furnished to Surency within thirty-one (31) days after the Child attains the age which would otherwise be disqualifying.
With respect to a person who is an Eligible Dependent on the Effective Date, coverage hereunder shall begin for such Eligible Dependent upon the later of i) the first day that the coverage commences for the Subscriber, or ii) the date such person satisfies the requirements to become an Enrollee.
No person may be insured both as an Eligible Employee and as an Eligible Dependent, and no person will be considered as an Eligible Dependent of more than one employee of the Employer.
