Eligibility Records definition

Eligibility Records means any and all documents utilized to make a determination as to whether a person is eligible to participate in the Georgia Volunteer Health Care Program.
Eligibility Records means the list submitted by Company or its designee to PBM indicating Members’ eligible to receive drug benefits under a Plan, as amended from time to time by Company in a reasonably accepted electronic format.

  • Eligibility Service of an employee means the period or periods of service credited to him under the provisions of Article II for purposes of determining his eligibility to participate in the Plan as may be required under Article III or Article VI.
