Eligibility and Enrollment definition

Eligibility and Enrollment. As of the first day of the term of this Contract, the City will have delivered to Florida Blue enrollment information regarding eligible and properly enrolled members, as determined by the City. The City shall deliver to Florida Blue all employee and dependent eligibility status changes on a monthly basis, or more frequently as mutually agreed by the parties. The City shall be responsible for providing each covered employee with a copy of the plan document which shall include the Group Health Plan.
Eligibility and Enrollment tab means the patient is not eligible under the Medicaid Program. The member is Medicare eligible and is responsible for any additional medical costs.

  • A Dependent's coverage will take effect on the day that the Eligibility and Enrollment Requirements are met and the Member is covered under the Plan or as otherwise stated in the Enrollment section of this Plan.

  • Number of Students Who Began the Program means the number of students who began a program who were scheduled to complete the program within 100% of the published program length within the reporting calendar year and excludes all students who cancelled during the cancellation period.

  • Open Enrollment Period means an enrollment period during which time any Eligible Employee and/or Dependent may apply to become a Subscriber and/or Covered Dependent, and existing Subscribers may apply to change to another provider network or coverage option, if available, or elect to terminate coverage.
