Dental Benefits definition

Dental Benefits means those dental procedures or services which are listed in this booklet subject to the exclusions, terms and conditions contained in this booklet.
Dental Benefits means benefits paid for those dental procedures or services covered under this group dental plan and subject to the exclusions, terms, conditions and provisions contained herein.

  • You will be responsible for any applicable deductible and coinsurance amounts listed in the Summary of Dental Benefits section.

  • Pediatric Dental Benefits Grievance ProcessMembers, a designated representative, or a provider on behalf of the Member, may contact the Dental Member Service Department by telephone, letter or online to request a review of an initial de- termination concerning a claim or service.

  • All other services not specifically included in this Plan as Covered Dental Benefits.DDWA shall determine whether services are Covered Dental Benefits in accordance with a standard dental practice and the Limitations and Exclusions shown in this benefits booklet.

More Definitions of Dental Benefits

Dental Benefits. Effective for the academic year 2022-2023, PhD Student Employees on appointment may enroll themselves and eligible dependents in the Emblem Preferred Dental Plan or an equivalent dental plan. The University will pay seventy-five percent (75%) of the monthly premium for PhD Student Employees and eligible dependents who enroll in the Emblem Preferred Dental Plan. The University will pay an amount equal to seventy-five percent (75%) of the monthly premium cost of the Emblem Preferred Dental Plan towards the monthly premium cost for Student Employees and eligible dependents who enroll in another dental plan that is offered to students. The structure of the Dental Plans shall not be a subject for collective bargaining. The University intends to offer the dental benefits described above to PhD Students who are not part of the bargaining unit and their eligible dependents. Vision Benefits: PhD Student Employees and eligible dependents will be eligible to participate in a vision plan that is offered to students.
Dental Benefits. The City shall continue to pay 100 % of the employee and eligible dependent’s dental coverage. Once a covered dependent is no longer eligible because of age limitations in the policies or deceased, said coverage shall cease. Dependent is defined as spouse or child who meets the criteria set forth by the medical insurance provider.

Basic Dental Services Charges up to the benefit maximum for: Oral exams, including scaling and cleaning of teeth, but not more than once every months; Periodontal scaling and/or root planning (limited to IO units per year for all procedures combined); Occlusal (limited to units per year); Topical applications of sodium or stannous fluoride but not more than one application every months; Dental x-rays, except that bite-wing x-rays are limited to one set every months; Fillings; Extractions; Oral surgery, including excision of impacted wisdom teeth; Antibiotic drug injections; Anaesthesia and its administration in connection with oral surgery or other covered dental services; Space including stainless steel crowns for primary teeth that have several cavities which would oth- erwise require fillings or which are non-restorable using normal restorative dental material; Repair, relining or of dentures; Repair, resurfacing or recementing of crowns, inlays, or bridges; Periodontic treatment for disease of the bone and gums of the mouth, including tissue grafts occlusal guards, but not athletic guards; Endodontic treatment, including root canal therapy. Major Dental Services Charges up to the benefit maximum for: Dentures: First installation, including adjustments of partial, per- manent or full temporary or permanent removable den- tures to replace or more natural teeth extracted while the person is insured; Denture adjustments that occur more than months after installation; Replacement of an existing partial or full removable den- tures, if it; was installed at least years before and cannot be made serviceable; or is a temporary full denture which replaces one or more natural teeth extracted...
Dental Benefits. The Plan provides dental benefits up to the prior years’ Ontario Dental Association suggested fee guide. Basic Dental Services are payable at Major Dental Services for dentures only are payable at Benefit Maximum: The maximum amount payable for Basic and Major Dental Services combined is per calendar year.
Dental Benefits means the group dental plan benefits established and maintained by the College and provided under the Plan, as amended from time to time.
Dental Benefits. The College proposes the same coverage as currently exists but does agree to freeze employee contributions to dental coverage for the length of the contract at the same level that was being paid at the time of contract ratification. The plan shall provide $10 copays for exams, $15 copays for single, bifocal, or trifocal lenses, $0 copay for necessary contact lenses, $130 allowance for frames, $130 allowance for elective contact lenses. Employee Assistance Program: A new Employee Assistance Program, i.e. an employee benefit program that assists employees with personal problems and work-related problems that may impact their job performance, health, mental and emotional well-being, shall be provided at College expense during the full-term of this contract.
Dental Benefits means the benefits provided for under the Policy in respect of dental expenses. Such expenses must be incurred by an Insured Person as a result of Injury, Dental Abnormalities or Conditions.
