Covered Expense definition

Covered Expense means expense incurred only for the following:
Covered Expense means expense incurred for services and supplies: (a) listed below; and (b) ordered or prescribed by a Legally Qualified Physician as Medically Necessary for diagnosis or treatment; which is limited to:
Covered Expense means a Usual and Customary fee for a Reasonable, Medically Necessary service, treatment or supply, meant to improve a condition or participant’s health, which is eligible for coverage in this Plan. Covered Expenses will be determined based upon all other Plan provisions. When more than one treatment option is available, and one option is no more effective than another, the Covered Expense is the least costly option that is no less effective than any other option.

Examples of Covered Expense in a sentence

  • The difference between the cost of a private Hospital room and the cost of a semi-private Hospital room is not a Covered Expense unless the Member’s Admission in a private Hospital room is Medically Necessary.

  • When benefits are reduced under a Primary Plan because a Member does not comply with the Primary Plan’s requirements, the amount of such reduction in benefits will not be a Covered Expense.

  • Incurred or Incurs means an obligation to pay for a Covered Expense for treatment, service or purchase of supplies, deemed to be the date it is provided to the Covered Person.

  • We pay up to the Maximum Covered Expense shown in the Schedule of Eye Care Services.

  • The remainder is the amount of Prescription Drug Covered Expense for that claim.

Covered Expense means a medically necessary service or supply which is usual, customary and reasonable, and which is listed for coverage in this Plan.
Covered Expense has the meaning set forth in Section 8.4.
Covered Expense means the medical, prescription drug, dental, or vision services and supplies shown as covered under the plan, including any applicable sales, excise, or other taxes.

All treatment is subject to benefit payment maximums shown in the Summary of Benefits and as determined elsewhere in this document.
Covered Expense means expenses actually incurred by or on behalf of a Participant for treatment, services and supplies covered by the Plan from a Provider. An eligible medical expense is deemed to be incurred on the date such treatment, service or supply, that gave rise to the expense or the charge, was rendered or obtained.
Covered Expense means the Reasonable and Customary charges for treatment, service or supplies. Such Expense shall not include any amount not customarily charged to persons without insurance.
Covered Expense means a charge for a service or supply allowable under the applicable Benefit Plan.
