Covered Dependent definition

Covered Dependent means any dependent of a Subscriber’s family, who meets and continues to meet all applicable eligibility requirements, and who is enrolled and actually covered under this Contract other than as a Subscriber.
Covered Dependent means a Dependent whose coverage is in effect. It does not include a Dependent whose coverage has ended.
Covered Dependent means a Dependent who (a) is listed in the documents necessary for coverage under the Contract, (b) has been accepted by Delta Dental for coverage, and (c) for whom the appropriate premium has been paid.

  • Notice of Ineligible DependentIf a Covered Dependent no longer meets all of the applicable eligibility requirements specified in the ELIGIBILITY AND ENROLLMENT FOR COVERAGE section of this Contract, the Contractholder must notify us in writing immediately and no later than 31 days after the date the Covered Dependent ceases to be eligible for coverage.

  • Benefits for loss of life of your Covered Dependent are paid to you.


More Definitions of Covered Dependent

Covered Dependent means an Eligible Dependent who is enrolled for Benefits under this Certificate and for whom Delta Dental has received Premium.
Covered Dependent means a Dependent who (a) is listed on the Application that is a part of this Policy;
Covered Dependent means a Dependent who (a) is listed on the Application that is a part of this Policy; (b) has been accepted by DDVA as a Covered Dependent; and (c) for whom the appropriate Premium has been paid.
Covered Dependent means a Covered Retiree’s dependent who meets the conditions for coverage under the EG&G Technical Services, Inc. Retiree Health Plan. In no event will the term Covered Dependent include any person who is an eligible Covered Retiree himself or any person who is employed full-time with the Employer. If both parents of any Covered Dependent child are eligible Covered Retirees, then the Covered Dependent child shall be considered as a Covered Dependent of only one of the Covered Retirees.
Covered Dependent means a Retired Participant’s Spouse or Domestic Partner at the time of retirement (as described in the definition of Retired Participant), and a child of the Retired Participant who meets the following conditions:
Covered Dependent means any Dependent who is covered under the Plan.
Covered Dependent means the Subscriber’s spouse or domestic partner, Subscriber’s biological child, Subscriber’s adopted child or step-child, or domestic partner’s biological or adopted child. (Coverage for adopted children of a Subscriber or domestic partner begins on the date on which the adoptive child’s birth parent or other appropriate legal authority signs a written document, including, but not limited to, a health facility minor release report, a medical authorization form, or a relinquishment form, granting the Subscriber, or the Subscriber’s spouse or domestic partner the right to control health care for the adoptive child, or absent a written document, on the date there exists evidence of the Subscriber’s or Subscriber’s spouse’s or domestic partner’s right to control the health care of the child placed for adoption.) The Plan shall not deny enrollment of a Subscriber’s child or a subscriber’s domestic partner children on any of the following grounds: (1) the child was born out of wedlock; (2) the child is not claimed as an exemption on the Subscriber’s federal income tax return; or (3) the child does not reside with the Subscriber or within The Plan’s service area. Dependent children are covered under the age of twenty-six (26). Dependent children who are incapable of self-sustaining employment by reason of a physically or mentally disabling injury, illness, or condition, and who are chiefly dependent upon the Subscriber for support and maintenance, are eligible for continuing membership in The Plan.
