Claim Statement definition

Claim Statement means an insurance policy, a document, or a statement made in support of or in opposition to a claim for payment or other benefit under an insurance policy, or other evidence of expense, injury, or loss. The term includes statements made orally, in writing, or electronically, including the following:

Claim Statement means the form delivered in accordance with this Claims Process in writing or digitally via the Claims Portal to each Claimant, by which they may confirm their Claim, substantially in the form attached as Schedule “C” hereto;

Examples of Claim Statement in a sentence

• Where an employee's vehicle is maliciously damaged as a direct result of the employee being employed by the Employer, the Employer shall reimburse the employee the cost of any deductible portion of insurance coverage on that vehicle up to $300 provided the employee can provide the Employer with a valid Police Number, and an Insurance Claim Statement.
