Accident Reports definition

Accident Reports are defined in subsections 4.2 through 4.4.
Accident Reports. When a youth is injured/hurt in the program, an AF Form 1187 Youth Flight Accident Report will be filled out informing parents of the injury. Once the AF Form 1187 is filled out, parents will be notified by the front desk clerks, or managers in the program and ask to sign upon pick-up. If parents desire a copy of the report, a FOIA request will have to be made at the FOIA office by parents. Accident Reports. All accidents that occur during the operation of motor coaches, including disturbances and ejectments, or connected with the operation of the Maintenance Division of the Employer regardless of their apparent insignificance, shall be fully, properly, and completely reported by the employee or employees involved, upon report blanks supplied by the Employer. Such reports shall be made and delivered during the day of such accident or other occurrence if possible, and if not, such reports must be made during the next regularly scheduled working day after the accident or other occurrence and shall be prepared in conformity with the Employer's rules. Employees will be allowed fifteen (15) minutes paid time for making out such accident reports when required to do so other than during their regular work hours.

Accident Reports. You will notify us immediately if the product is lost, stolen, damaged, destroyed, or if any person is injured or dies or if any property is damaged as a result of use of the product.   • Adverse Event means any untoward medical occurrence in a patient or clinical investigation subject administered a pharmaceutical product and that does not necessarily have a causal relationship with the treatment. A reasonable expectation of death or serious injury before abatement exists if a rational person, subjected to the same conditions or practices giving rise to the peril, would not expose the person's self to the danger during the time necessary for abatement.
