Accident Notification. If in the course of completing work as part of this Agreement there is an accident that involves the public, CONTRACTOR shall as soon as possible inform the COUNTY of the incident by telephone. CONTRACTOR shall follow up in writing within two (2) business days of the incident. If Law Enforcement was involved and has written a report, CONTRACTOR shall forward a copy of the report to the COUNTY.
Accident Notification. CONTRACTOR shall be responsible for all injuries, accidents and other mishaps associated with its operations. CONTRACTOR shall report any accidents resulting from the performance of this AGREEMENT to AGENCY as soon as practicable by telephone or messenger. For purposes of this Section, "accident" shall include the death of any person, any personal injury resulting in inpatient hospitalization or out-patient treatment by a physician, or damage to any real or personal property exceeding $10,000. CONTRACTOR shall report, in writing, to AGENCY, within seven (7) days of knowledge that the accident exceeds ten thousand dollars ($10.000), complete details of the accident, including witness statements. Any violation of this reporting requirement shall constitute a minor breach.
Accident Notification. The Health and Safety Committees shall be notified in writing of each lost time accident or injury. The Employer shall investigate and report in writing to the Safety Committees as soon as possible on the nature and cause of the lost time accident or injury.
Accident Notification. In the event of accidents of any kind which involve the general public and/or private or public property, as related to services provided herein, the Contractor shall immediately notify the City and shall provide a full accounting of all details of the accident. Contractor shall furnish the City with copies of all reports of such accidents at the same time that the reports are forwarded to any other interested parties.
Accident Notification. Buyer shall provide Seller with prompt notification of any accident or similar event that may involve Goods delivered under an Order and shall allow Seller to participate in any investigations, hearings, or related activities to the extent such Goods are involved.

Notify the Owner as soon as practical, but not later than four hours, after any accident meeting the definition of Recordable Injuries or Illnesses or High Visibility Accidents, property damage equal to or greater than $2,000, or any weight handling equipment accident. Information shall include contractor name; contract title; type of contract; name of activity, installation or location where accident occurred; date and time of accident; names of personnel injured; extent of property damage, if any; extent of injury, if known, and brief description of accident (to include type of construction equipment used, PPE used, etc.). Preserve the conditions and evidence on the accident site until the Owner investigation team arrives on-site and Owner investigation is conducted.
Accident Notification. In the event of accident resulting in damage to persons or property which, arise from the performance of this Agreement, Contractor shall promptly notify the City. Contractor shall disclose to the City the identity of the persons involved in the accident, and provide a general description of the physical injuries to persons and/or damage to property occasioned in the accident. Contractor agrees to cooperate with its insurance companies in the investigation and resolution of any claims arising from any such accident.
Accident Notification. The Co-Chairs of the Health and Safety Committee shall be notified of each accident or injury and report to the Union and the Employer as soon as possible on the nature and cause of the accident or injury.
