This Family Transportation Agreement is entered into by and between the Department of Elementary and Secondary Education as the Part C Lead Agency, hereinafter referred to as DESE and: _________________________ ( list the name of the person to be paid), hereinafter referred to as the Payee. This Agreement is made on the behalf of the Missouri Department of Health and Senior Services, Bureau of Special Health Care Needs, hereinafter referred to as DHSS; the Department of Mental Health, Division of Mental Health and Developmental Disabilities, hereinafter referred to as D MH, and the Department of Social Services, Division of Medical Services, hereinafter referred to as D MS, for the purposes of coordination and collaboration in the provision of services to eligible children and their families under Missouri’s early intervention system.
