To become authorized as a non-contract provider please complete this form. Services cannot be performed under the non-contract provider authorization agreement until it has been reviewed and approved by NYSCB. If there is no other effective agreement (i.e., an Interconnection Agreement) between WSP and CenturyLink concerning access to Poles, Ducts and ROW, then this Agreement/Attachment 3 must be executed by both parties in order to start the Inquiry Review and in order for WSP to obtain access to Poles, Ducts and/or ROW.
