This agreement with the University of Pikeville and it’s Kentucky College of Osteopathic Medicine (hereafter KYCOM), is solely intended to provide educational training opportunities for KYCOM students. With this affiliation, I agree to appointment as a KYCOM Adjunct Clinical Faculty or Guest Lecturer effective from the date signed below.
