This Restricted Stock Purchase Agreement (the “Agreement”) is made and entered into as of the date specified on Carta (the “Effective Date”) by and between PlushCare, Inc., a Delaware corporation (the “Company”), and the purchaser indicated on Carta (“Purchaser”), by their mutual acceptance of this Agreement via Carta. Capitalized terms not defined herein shall have the meanings ascribed to them in the Company’s Stock Incentive Plan, as may be amended from time to time (the “Plan”).
