This Local Exchange Telecommunications Services Resale Agreement (“Agreement”), is effective as of the day of 199_ (the “Effective Date”), by and between Cincinnati Bell Telephone Company, an Ohio corporation with offices at 201 E. Fourth Street, Cincinnati, Ohio 45201 (“CBT”), and Akiva T. Hunter dba EZ-Tel Communications with offices at 1038 Caliente Drive #23, Jacksonville, FL 32211 (“EZ-Tel”).
