This Memorandum of Agreement (MOA), effective December 1, 2017, facilitated by the Healthy Start Coalition of Hardee, Highlands and Polk Counties, Inc. ("Healthy Start"), and CI&R Partners will serve as a basis for a collaboration in providing home visiting and referral services for families in Hardee, Highlands and Polk Counties.
