This Amendment is hereby entered into by and between Company Care, 5000 N. 26th Street, Suite 200, Lincoln, NE 68521 (hereinafter “Contractor”) and City of Lincoln and Lancaster County, (hereinafter “Owners”), for the purpose of amending the Contract dated October 21, 2013 executed under City Executive Order No. 86566, and County Contract C-13-0549, dated November 5, 2013 for RFP - Employee Medical Exams, Bid No. 13-197, which is made a part of this amendment by this reference.
