Withdrawal of Authorization. If the use or disclosure of PHI in this Agreement is based upon an Individual’s specific authorization for the use or disclosure of his or her PHI, and the Individual revokes such authorization, the effective date of such authorization has expired, or such authorization is found to be defective in any manner that renders it invalid, Business Associate shall, if it has notice of such revocation, expiration, or invalidity, cease the use and disclosure of the Individual’s PHI except to the extent it has relied on such use or disclosure, or if an exception under the Privacy Rule expressly applies.
Withdrawal of Authorization. If the use or disclosure of PHI in this Agreement is based upon an individual’s specific authorization for the use of his or her PHI, and (i) the individual revokes such authorization in writing, (ii) the effective date of such authorization has expired, or (iii) the consent or authorization is found to be defective in any manner that renders it invalid, Business Associate agrees, if it has notice of such revocation or invalidity, to cease the use and disclosure of any such individual’s PHI except to the extent it has relied on such use or disclosure, or where an exception under the Confidentiality Requirements expressly applies.
Withdrawal of Authorization. The Lender may withdraw this authorization given to the Borrower if the Borrower is in default under this Contract.
Withdrawal of Authorization. If an Individual’s specific authorization is required for use or disclosure of PHI in this Business Associate Agreement, and the Individual revokes such authorization, the effective date of such authorization has expired, or such authorization is found to be defective in any manner that renders it invalid, CA-CIB shall, if it has notice of such revocation, expiration, or invalidity, cease the use and disclosure of the Individual’s PHI except to the extent it has relied on such use or disclosure, or if an exception under the Privacy Rule expressly applies.

If the use or disclosure of PHI in this BAA is based upon an Individual’s specific authorization for the use or disclosure of his or her PHI, and the Individual revokes such authorization, the effective date of such authorization has expired, or such authorization is found to be defective in any manner that renders it invalid, Business Associate shall, if it has notice of such revocation, expiration, or invalidity, cease the use and disclosure of the Individual’s PHI except to the extent it has relied on such use or disclosure, or if an exception under the HIPAA Regulations expressly applies.
Withdrawal of Authorization. An insurance undertaking must immediately notify the Swiss supervisory authority if its authorization to conduct business in Liechtenstein has been withdrawn.
Withdrawal of Authorization. An individual’s authorization is not required when PHI is being used for accreditation purposes pursuant to a business associate agreement. However, if the use or disclosure of PHI in this Addendum is based upon an Individual’s specific authorization, and the Individual revokes such authorization in writing, or the effective date of such authorization has expired or is found to be defective in any manner that renders it invalid, FACT agrees, if it has notice of such revocation or invalidity, to cease the use and disclosure of any such Individual’s PHI except to the extent it has relied on such use or disclosure, or where an exception under the HIPAA Privacy Standards expressly applies.

Knowledge of HIPAA Rules. Business Associate agrees to review and understand the HIPAA Rules as it applies to Business Associate, and to comply with the applicable requirements of the HIPAA Rule, as well as any applicable amendments.
Withdrawal of Authorization. Hitachi reserves the right, in its sole discretion and for any reason, to withdraw its authorization of Dealer’s right to conduct Internet-based sales of any or all of the Products at any time by giving written notice to Dealer.
