VISION PROGRAM. The City shall provide a vision plan for employees and dependents. The City shall pay the premium. The plan coverage shall be as indicated on provider contract documents on file in Human Resources.
VISION PROGRAM. Effective July 1, 2001, the Board shall provide vision coverage in accordance with the schedule of benefits that is attached as Appendix F.
VISION PROGRAM. This section concerns video display terminals (VDT), eye examinations and eyeglasses/contacts (including disposable lenses) and is entered into by and between SSA and the AFGE General Committee. This Agreement covers all employees in the AFGE bargaining unit that use a VDT as part of their normal work.
VISION PROGRAM. Vision program is self-insured by the Authority that reimburses employee and family members up to $175 per family member per calendar year. For all Union employees, full reimbursement for prescription safety glasses will be made every two (2) years.
VISION PROGRAM. The Employer shall make available a vision care program which provides coverage for employees only. An employee may be reimbursed up to $130 over the one-year term of this Agreement for the costs of an examination by a licensed vision care practitioner and the purchase of eyeglass lenses and frames. Reimbursement shall be made subsequent to the submission to the Business Office of a receipt for the examination and/or lenses or frames that is less than one (1) year old. Once an employee has been reimbursed a total of $130 or the costs of exams, glasses, and/or frames, the employee shall not be entitled to any additional reimbursement for such purchases for the balance of the one year Agreement. In the event that the parties decide to continue the terms and conditions of the 2020-2021 Agreement while they bargain their successor agreement, the parties agree that they will make a preliminary determination of the vision benefits to be made available to the employees during the pendency of their negotiations. In order to be reimbursed for vision care, all receipts must be turned in to the Business Office no later than one year from the date of vision care.
VISION PROGRAM. Upon the legal execution of the labor agreement, the City shall provide the following vision plan per covered employee: $40 benefit payable each calendar year for an exam (including refractions) and $150 benefit per calendar year for eyeglasses, frames, contact lenses. In the event that dependent vision coverage is determined to be a prevalent benefit in the comparability determination as referenced in this agreement, such coverage, as stated above, shall apply to covered employee’s eligible dependents effective the first pay period of the 2011 payroll year.

VISION PROGRAM. The Board shall provide a comprehensive Vision Program for each member of the bargaining unit and their families who are eligible and make application for such insurance. The Board shall pay 80% of the full cost of the program, and any increase thereafter. Amount of coverage will be increased for one (1) eye exam once every twelve (12) months.
