User Safety. Although the USER will be informed regarding general safety policy prior to being allowed to use the facilities, the USER assumes responsibility for his/her own personal safety. It is expected that the USER will operate all instruments and equipment in a safe and professional manner, consistent with the operating instructions and the facility polices. The USER represents that his/her knowledge of laboratory practices is adequate to permit the safe pursuit of the research work in conjunction with the USER’s specific project. The UNIVERSITY and HCBI shall have the right to immediately prohibit further use by the USER if HCBI, in its sole discretion, believes that the USER has breached this representation.

You should not provide your financial information (for example, your credit card or bank account information), or wire or otherwise send money, to other users. YOU ARE SOLELY RESPONSIBLE FOR YOUR INTERACTIONS WITH OTHER USERS. YOU UNDERSTAND THAT CLICKED DOES NOT CONDUCT CRIMINAL HISTORY CHECKS ON ITS USERS OR OTHERWISE INQUIRE INTO THE BACKGROUND OF ITS USERS. CLICKED MAKES NO REPRESENTATIONS OR WARRANTIES AS TO THE CONDUCT OF USERS. CLICKED RESERVES THE RIGHT TO CONDUCT ANY CRIMINAL BACKGROUND CHECK OR OTHER SCREENINGS (SUCH AS SEX OFFENDER REGISTER SEARCHES) AT ANY TIME USING AVAILABLE PUBLIC RECORDS. BY AGREEING TO THIS AGREEMENT, YOU HEREBY AUTHORIZE ANY SUCH CHECK.

While UNM will inform User regarding general safety policy prior to being allowed to use the laboratory facilities, User assumes primary responsibility for the personal safety of its employees/agents. UNM expects that User will operate all instruments and equipment in a safe and professional manner, consistent with the Operating Procedures. User warrants that its employees’ and agents’ knowledge and training is adequate so that they can safely conduct the work under User’s specific research project, or that they shall request appropriate assistance from UNM personnel. UNM shall have the right to immediately prohibit further use by any employee or agent of User if UNM, in its sole discretion, believes that User has breached this representation with regard to any particular employee or agent. User shall not bring onto any UNM facility chemicals or other hazardous materials without first obtaining written consent of the facility manager. User shall disclose in writing all such chemicals and other hazardous materials brought onto the UNM facility, including amounts, dates, and disposition. User agrees to abide by UNM policies on the storage, handling and disposition of all hazardous materials.

It is expected that User will operate all instruments and equipment in a safe and professional manner, consistent with the safety procedures, operating instructions and rules of the laboratory facilities. User represents that its employees’, agents’ and associated individuals’ knowledge of microfabrication processing and cleanrooms is adequate to permit the safe pursuit of the research work in conjunction with the User’s specific project. Vanderbilt shall have the right to immediately prohibit further use by any User employee, agent or associated individual, if Vanderbilt, in its sole discretion, believes that User has breached this representation with regard to any particular employee, agent or associated individual. User will be provided with certain safety information from Vanderbilt prior to User being authorized to use the Research Facilities. User must review such information and certify in writing that it understands said information and has been given full opportunity to ask Vanderbilt any questions it may have about such information. User understands that all of its employees, agents and associated individuals must complete all required training before being granted privileges to access or use the Research Facilities. No employee, agent or associated individual who has not received the required training will be allowed to access the Research Facilities except for the purpose of receiving the required training. User’s authorized official and each of User’s employees, agents and associated individuals shall certify in writing their understanding and agreement to comply with the aforementioned informational and training requirements by completing the form contained in Attachment B hereto. User’s completed Attachment B forms are hereby incorporated into this Agreement.
