User Participation. Every individual or entity that wishes to participate as a user of PRIR must sign and comply with the security agreement and confidentiality PRIR. Any system user who violates the confidentiality and security agreement is subject to being revoked access permission and may result in civil and / or criminal for making improper use of confidential information. The PRIR’s user security and confidentiality must be signed by an authorized health care institution or the educational institution prior to any training in use and access to the database representative. At least one person from each entity must complete the training PRIR before obtaining access permission. Access to the system must be granted by the Data Manager of the Immunization Program. The data manager maintains a record with the security and confidentiality agreements signed by system users. These agreements must be renewed every year. Unlimited access will only be given to system personnel functions to provide vaccination services. All personnel including permanent, temporary, volunteers, contractors and consultants will be required to sign a confidentiality agreement and security before gaining access to registration information. When an employee resigns or is transferred to work your permission access will be suspended immediately. Any employee who requests any license exceeding six months should notify the data administrator at least one day in advance and your account will be given an inactive coding. Users who do not use their account for more than 60 consecutive days their accounts will be automatically inactivated. Access to the register will only be allowed through established processes. Each person who is granted access must have an identification coding and a counter signal. Sharing this identification and password will not be allowed. Users are prohibited from disclosing information about access codes or protocols of the system to unauthorized persons. Data administrators shall ensure that users have been adequately trained to use the system properly and do not have a higher level of access required to perform their required duties. Classified information will only be accessible to the staff of the Department of Health, its agents or authorized users. User Participation sessions approved by Xxxx, or session Terms modified by Xxxx and agreed to by the Rifter, may be advertised through the services. When a session is advertised through the services, end users of the services may elect to participate in the session. All sessions approved by Rift may be cancelled by Rift at any time. The Rifter will not receive the fee designated in our published fee policy for any sessions (i) cancelled by Xxxx, (ii) having zero participants, or (iii) not occurring according to the session Terms.

User Participation. Every individual who wishes to participate as a user of the Registry must read this Organization Security and Confidentiality Agreement and sign and comply with the Registry User Security and Confidentiality Agreement (DPH F-00103). The Department administers and controls access to the Registry and the data contained therein. Any use of the Registry that violates the User Security and Confidentiality Agreement will subject the user to revocation of the user’s access privileges and may result in civil or criminal penalties for improper disclosure of health information under Wis. Stat. sec. 146.84. In addition, egregious or repeated violations by an Organization’s users may result in revocation of the Organization’s access privileges. The Registry Organization Security and Confidentiality Agreement must be signed by a representative of the Organization prior to any training of Organization staff on use of the Registry or Organization access to the Registry data. This person shall serve as the Organization’s Site Administrator, and shall have responsibility for all individual users within the Organization. The Site Administrator shall complete the Department administered training for Registry Site Administrator. Having completed the training, the Site Administrator may enroll users who have been trained in the use of the Registry at the appropriate access level and have signed the Registry User Security and Confidentiality Agreement. The Site Administrator shall maintain a file of signed Registry User Security and Confidentiality Agreements and ensure that each user within the Organization renews his or her agreement every two years. Users with expired agreements or revoked privileges shall not access the Registry. The Site Administrator shall be responsible for each user within the Organization. Only personnel whose assigned duties include functions associated with the blood lead testing of children may be given access to Registry information. All such personnel, including permanent and temporary employees, volunteers, contractors, and consultants shall be required to sign a Registry User Security and Confidentiality Agreement before gaining access to the Registry.
