Unpaid Medical Leave. 1. Upon written notice of a bargaining unit member, the Board shall grant an unpaid medical leave of absence for a period of not more than two (2) years where illness, dependent care or other disability is the reason for the request. Such leave shall be granted by the Board in up to one (1) year increments.

Unpaid Medical Leave. A unit member who becomes medically disabled and who has exhausted his/her sick leave or who elects not to use his/her sick leave may request and shall be granted unpaid medical leave. A unit member's application for unpaid medical leave shall be accompanied by a physician's statement describing the nature of the medical disability, its expected duration, and the period of time the unit member should be relieved of his/her duties. This leave shall not exceed two (2) consecutive school years. The unit member while on leave shall have the right to pay the total premium for group life, hospitalization, and other group insurance benefits, provided payment is acceptable to the insurance carriers. All payments shall be made on or before the date established by the Board Treasurer.

Unpaid Medical Leave. SECTION 33.1 - The Board will utilize the provisions contained in ORC 3319.13 for authorizing an unpaid medical leave of absence for bargaining unit members, either voluntarily requested by a bargaining unit member or involuntarily initiated by the Board.
Unpaid Medical Leave. The Superintendent is authorized to grant unpaid medical leave. The leave may not extend beyond eighteen (18) months and shall be without salary or District paid health care benefits except as required under FMLA/OFLA. The employee will be entitled to purchase district health insurance coverage for the duration of the leave. The starting date of the leave shall be determined upon mutual agreement between the Superintendent and the staff member so the effective time of the leave is both in the best interest of the school system and consistent with the health and welfare of the teacher. During the leave the staff member shall retain his or her position on the salary schedule and all accumulated benefits. Should the leave be for a period greater than five (5) school months, no accumulated sick leave will be granted for that year and no increment shall be earned on the salary schedule except if the leave is for a job related injury. When returning to duty, the teacher will be assigned to his or her former position, if possible, or to a position of like nature.
Unpaid Medical Leave. When a Teacher exhausts all full and partial paid sick leave, and he/she is unable to return to work, he/she must submit a request for medical leave, or an extension of medical leave, for a specified period of time accompanied by a statement of verification from a physician, to the District Privacy Officer. This leave can be used for the illness of a family member as previously defined. Such leave may be granted for a period not to exceed twelve (12) months, and shall be without pay. At least ten (10) business days prior to a Teacher returning to work, a physician's statement of release must be submitted to the District Privacy Officer.
Unpaid Medical Leave. A. Employees shall be entitled to family and medical leave in accordance with the Family and Medical Leave Act of 1993, as amended.

Unpaid Medical Leave. 23.3.2.1. If a unit member continues to be medically unable to perform the duties of his or her position after exhausting all available sick leave, including accumulated sick leave and the five month extended period, and is not medically able to resume the duties of his or her position, the District will place the unit member on the reemployment list for 24 months, if probationary, and 39 months, if permanent. Should the unit member become medically able to resume his or her duties while on the reemployment list, he or she shall be returned to employment in a position for which he or she is credentialed and qualified. The 24-month or 39-month period shall commence at the expiration of the five-month period.
Unpaid Medical Leave. A Bargaining Unit Member who runs out of sick leave shall be granted an unpaid leave of absence for the duration of the illness or disability. An unpaid leave of absence may be granted for those eligible pursuant to Article 13-1-i. Other serious family illness shall be allowed with the recommendation of the supervisor and approval of the Superintendent.
Unpaid Medical Leave. A. The University complies with and exercise its rights under and pursuant to the Family and Medical Leave Act and the Americans with Disabilities Act (as amended). Therefore, the initial portion of this leave all shall exhaust the employee's FMLA entitlement (i.e., FMLA leave runs concurrently with leave under this article), and all entitlement to paid leave will be used before any unpaid leave commences.
