UNIVERSITY OF MINNESOTA EMPLOYEE INSURANCE PLAN. (UPlan) During the life of this Agreement, the Employer agrees to offer a Group Insurance Program that includes medical, dental, life with matching accidental death and dismemberment, and disability coverages. The UPlan will make a UPlan Summary describing these coverages available to all insurance eligible employees. The UPlan Summary shall be provided no less than biennially and prior to the beginning of the insurance year. New insurance eligible employees shall receive a UPlan Summary within thirty (30) days of their enrollment in the plan.

UNIVERSITY OF MINNESOTA EMPLOYEE INSURANCE PLAN. During the life of this Agreement, the Employer agrees to offer a Group Insurance Program that includes medical, dental, life, accidental death and dismemberment, and disability coverages. For the remainder of the 2001 insurance contract year, the Employer will provide the coverage and contributions in effect as of June 30, 2001. For the 2002 insurance year the Employer will provide medical coverage under a new set of plans, separate from the State Employees Group Insurance Program. Dental, life, accidental death and dismemberment, and disability coverages will continue to be provided through the State Employees Group Insurance Program Beginning with the 2003 insurance year, the Employer may withdraw all participation from the State Employees Group Insurance Program, and contract directly with carriers to provide these coverages for its employees. All insurance eligible employees will be provided with a Summary Plan Description describing these coverages. Such Summary Plan Description shall be provided no less than biennially and prior to the beginning of the insurance year. New insurance eligible employees shall receive a Summary Plan Description within thirty (30) days of their date of eligibility.

  • Ontario Health Insurance Plan The parties recognize that the method of funding OHIP has been changed from an individually paid premium to a system funded by an employer paid payroll tax. If the government, at any time in the future, reverts to an individually paid premium for health insurance, the parties agree that the Colleges will resume paying 100% of the billed premium for employees.

If Contractor does not have employees, it shall provide a letter, on company letterhead, to the Judicial Council certifying, under penalty of perjury, that it does not have employees.   • GROUP INSURANCE PLANS 15.01 The Company agrees to provide all full time employees with one (1) or more years of continuous service, a *Short Term Disability Benefit (S.T.D.) as generally described in Section B of a notice to all employees of the amended Benefit Plan dated May 1, 1993. *Payment for Short Term Disability shall begin on the third (3rd) consecutive day of absence. For those employees who have completed ninety (90) days of perfect attendance from the last date of return to work from an absence due to sickness or accident, shall be paid S.T.D. from the first (1st) day for the first covered absence following the ninety (90) days of perfect attendance.
