Treatment Xxxxxx Care. A clinically effective alternative to residential treatment facilities that combines the treatment technologies typically associated with more restrictive settings with a nurturing and individualized family environment.

Treatment Xxxxxx Care. 2.5.9.11.1. The Contractor shall demonstrate an adequate number of Providers who can provide treatment for children in xxxxxx care, which includes clinicians who are experienced in working with the child welfare system. This shall be reported on the network adequacy reports by indicating Providers who are proficient with Treatment Xxxxxx Care.

Treatment Xxxxxx Care. A service that provides clinical intervention for a Class Member within the private homes of clinically trained and licensed xxxxxx families for the length of time necessary to meet the individual treatment needs of the Class Member. ICC will be provided when a Class Member is placed in treatment xxxxxx care and the CFT members will include the treatment xxxxxx parents. Treatment xxxxxx care includes services provided by a xxxxxx parent/family in order to implement the Class Member’s individualized treatment plan. Treatment xxxxxx parents assist in developing an individualized treatment plan for the Class Member and support the Class Member in achieving his/her service plan goals and objectives. Treatment xxxxxx parents perform a therapeutic function in addition to supervision services. Treatment xxxxxx care services include supervision, behavioral interventions, psychosocial rehabilitation, skills training and development, participation in treatment and discharge planning, and transition services when a Class Member returns to his/her family. Transition services involving the treatment xxxxxx parents may include, among other things, facilitating visits, coaching the permanency caregivers, providing limited respite care, etc. Class Members in treatment xxxxxx care may also receive other services listed in this document that are not provided by their treatment xxxxxx parents.

Treatment Xxxxxx Care. Worker Activities 1. Identify services to be provided to the parent(s), the child(ren), relative caregiver and to treatment xxxxxx parents by the treatment xxxxxx care worker. State activities that support the services offered to all participants in the service plan. 2. State proposed treatment xxxxxx care worker contact with the family, child(xxx), caretakers, and service provider, if applicable. 3. If the youth is age 14 or older, detail the independent living preparation activities the worker will provide to assist the youth. (See FOM 722-6 Independent Living Preparation.) 4. Identify what the treatment worker will do to facilitate parenting time and sibling visitation, if applicable. 5. If siblings are in separate placements, identify the ongoing efforts the xxxxxx care worker will make to place the siblings within the same home. 6. Identify all required treatment xxxxxx care worker actions to ensure educational stability for the child. 7. Document all efforts to identify and locate absent parents and/or relatives. 8. Document ongoing efforts to engage the child, caregiver, and family in services. 9. Indicate worker availability and describe crisis plan. 10.
