Transitional Support Services. The district will provide the following support services, for up to 90 days after case closing, to individuals whose TA cases have closed due to employment: To the extent Federal, State and Local resources are available, BCDSS will provide suppo rtive services deemed necessary and reasonable to assist employed Temporary Assistance recipients improve their self - sufficiency opportunities. This will be accomplished by referral for separate determination of eligibility for transitional medical/childcare services. Supportive services as outlined in attached supportive services desk guide will be provided as requested.
Transitional Support Services. The district will provide the following supports and strategies to support job retention: HRA’s employment vendors provide participants with job retention case management services for a number of months following job placement. Employment program vendors generally provide such services for a minimum of one year. Many vendors also have the option of providing several months of free transportation (up to three months) to improve job retention and to help ease the transition from welfare to work. HRA generally seeks to include in its employment vendor contracts incentives for successful job retention efforts. The district will provide the following support services, for up to 90 days after case closing, to individuals whose TA cases have closed due to employment: It is an HRA priority to help those who move off Temporary Assistance on account of employment to continue to succeed by supporting their efforts to stay employed and offering them services to allow them to improve their skills and get better jobs. Job retention and job upgrade services are vital to the success of HRA’s efforts to help individuals and families achieve self-sufficiency. If a Temporary Assistance case is closed due to employment-related reasons, family members will automatically receive 12 months of Medicaid coverage from the time the case is closed provided the family continues to have a dependent child and meets other eligibility requirements. In addition, the district provides a Medicaid Plan of Self Support that enables childless Safety Net Assistance (SNA) participants with special needs, who become ineligible for continued Temporary Assistance because their income from employment is sufficient to meet their needs, to continue ongoing medical, mental health or substance use disorder treatment. Medical assistance can continue for up to one year after an SNA special needs case is closed due to employment earnings. If a Temporary Assistance case with children is closed due to employment-related reasons, the family will automatically continue to receive SNAP benefits for up to five months at no less than the SNAP benefit amount received prior to case closing. Families and individuals on Temporary Assistance cases without children closed due to employment-related reasons will be eligible for SNAP benefits in accordance with normal budgeting and eligibility rules. Transitional Support Services. The district will provide the following supports and strategies to support job retention: MCDSS has built paid performance incentives for retention into all job placement and job development contracts to encourage placement contractors to provide case management and additional retention services including financial literacy, VITA services, etc. MCDSS works closely with Child Support Enforcement to help our families get the support in place to help transition from welfare to self-sufficiency. MCDSS utilizes TEAP to encourage employers to work with participants to achieve better outcomes. The Self Sufficiency Workgroup is equipped to intervene if an employee is struggling, before the client is terminated. The district will provide the following support services, for up to 90 days after case closing, to individuals whose TA cases have closed due to employment: MCDSS will determine eligibility for diversion payments during the 90 day period after case closing due to employment. If eligible, payments will be issued to prevent loss of employment or return to Temporary Assistance.MCDSS has also contracted for an aftercare program "Paths to Empowerment" will link individuals closed for excess income to resources and assistance in the community for 6 months. These services can be but are not limited to-financial literacy education, transportation assistance, rental assistance, basic life skills education.

The district will provide the following supports and strategies to support job retention: The District will provide Child Care Subsidy, Supportive Services, and Diversion Payments to support job retention. Please see the Supportive Services Plan (Attachment F). The district will provide the following support services, for up to 90 days after case closing, to individuals whose TA cases have closed due to employment: The following supportive services will continue to be available for 90 days after case closings: child care subsidy; motor vehicle repair; insurance, and registration; transportation; tools and equipment; job related safety equipment/clothing; and driver's license and permits. The District may also provide any other supportive services, within regulation, which are deemed necessary to allow individuals to participate in work activities, assist individuals at risk of needing Temporary Assistance to improve their opportunities for employment or to maintain their employment, or to assist employed Temporary Assistance recipients to improve their opportunitiies for employment which will move them to self-sufficiency. Please see the Supportive Services Plan (Attachment F).
Transitional Support Services. Concurrent with the Closing, the parties will execute a Transition Services Agreement.

Support Services will be explained and the LDSS worker will inform the client on how to contact the agency when they have any questions and or to provide case management services. The agency will also send a congratulation letter to the employed individual which will include information regarding support services, transitional supports and other local agency and community based organizations phone numbers that may be a viable resource to the employed individual. The district will provide the following support services, for up to 90 days after case closing, to individuals whose Temporary Assistance cases have closed due to employment: Clothing and personal needs, public transportation or other means of transportation, car insurance and repairs, driver's license/permit and or vehicle registration/title and fees, professional license, drivers education, tuition, employment counseling; any other allowable expenses as deemed necessary and reasonable by the LDSS to maintain employment. Approval of any and all requests is subject to availability of Federal, State, andlocal funds.
Transitional Support Services. The district will provide the following supports and strategies to support job retention: Any supportive service listed in 4.1, with the exception of gas vouchers or mileage reimbursement and costs of employer-specific prerequisite training expenses. For individuals with vehicles we can provide a gas card to assist with gasoline expenses for the TANF population. In addition to the supportive services previously described, the department will assist persons with transitional SNAP benefits, transitional Medicaid, child care and emergency assistance as eligibile. The district will provide the following support services, for up to 90 days after case closing, to individuals whose Temporary Assistance cases have closed due to employment: Agency will provide transitional Medicaid and transitional SNAP benefits, the same as noted above
