THIRD INCIDENT. Following the third incident resulting in damage to the device, written communication regarding the incident will be directed to the parent or guardian of the student to whom the device was issued, and a meeting will take place between the student and the appropriate Principal. The Principal may issue discipline to the student at his or her discretion. Fees for repair or replacement of the device, according to the schedule listed on Page 5 of this Policy, will be assessed to the student’s account and shall be the responsibility of the parent or guardian.

THIRD INCIDENT. You will be allowed to use a device only at school, and the device must be checked in and out with the assigned building staff at the beginning and end of the school day. You will not be allowed to take the device home for the remainder of the school year. You may face school disciplinary actions. You and your family may be held responsible for the full cost of repair or replacement. If you fail to return the device and/or related components at theend of the school year, upon withdrawal or when directed to doso, the device and related components will be considered stolen property. In that case, the district will not release your records and will le a police report. You must return the district- provided charging accessory and any other device related items issued by the district when you return your device. Replacement Costs for Damages and Repairs: Screen Repair $120 Device Replacement $380 Misc. Hardware $200 Note About Insurance The Xxxxxxx Valley School District strongly recommends that you purchase insurance for your child’s device and the school district offers optional yearly insurance allowing students the ability to take their devices home daily. If you have homeowner’s insurance or rental insurance, you may also be able to add a device to your insurance. Please note families are responsible for replacement if you choose not to purchase insurance and your device is lost, stolen, damaged outside of acceptable use, etc. Insurance for the device is a yearly cost and does not cover the charging accessories or protective cover.
THIRD INCIDENT. Member is suspended or expelled from the program or Fitness Center. If a minor, parent will be required to sign suspension notice. If an employee, Personnel Guidelines will be enforced. The severity of the incident or incidents, will dictate the severity of the consequences and the Lake Forest Fitness Center reserves the right to determine the amount of refund, if any. Members 12 & 13 years old are required to be directly supervised by their own parent or guardian in the Fitness Center at all times. The Lake Forest Recreation Department reserves the right to restrict, suspend, expel or deny participation in any program or service to any person whose behavior materially interferes with or disrupts the quality or the enjoyment by other participants, or the ability of staff to conduct or manage the activity or facility. Unacceptable Behaviors include but are not limited to: • Use of abusive language or language that is demeaning to others • Use of physical force against participants or employees • Disrespect towards other participants or employees • Damage to or inappropriate use of property, equipment or facility • Behavior that is disruptive or against the law • Any other behavior that the Recreation Staff feels is inappropriate or dangerous In the event of any emergency, I authorize the Lake Forest Recreation Department to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed reasonable and necessary for myself or my minor child’s immediate care and agree that I will be responsible for payment of any and all medical services rendered. I have read and fully understand and agree to the above Liability Waiver and Hold Harmless Agreement.

  • Data Incidents Merchant must report all instances of a Data Incident (as defined in the American Express Merchant Operating Guide) immediately to ISO after discovery of the incident.   • Reporting Unsuccessful Security Incidents Business Associate shall provide Covered Entity upon written request a Report that: (a) identifies the categories of Unsuccessful Security Incidents; (b) indicates whether Business Associate believes its current defensive security measures are adequate to address all Unsuccessful Security Incidents, given the scope and nature of such attempts; and (c) if the security measures are not adequate, the measures Business Associate will implement to address the security inadequacies.

  • Security Incident Notification The Transfer Agent shall promptly notify the Trust but in no event later than 72 hours following discovery of any Security Incident(s). Such notification shall include the extent and nature of such intrusion, disclosure, or unauthorized access, the identity of the compromised Customer Confidential Information (to the extent it can be ascertained), how the Transfer Agent was affected by the Security Incident, and its response to such Security Incident. The Transfer Agent shall use continuous and diligent efforts to remedy the cause and the effects of such Security Incident in an expeditious manner and deliver to the Trust a root cause analysis and future incident Mitigation plan with regard to any such incident. The Transfer Agent shall reasonably cooperate with the Trust’s investigation and response to each Security Incident. If the Trust determines in its sole discretion that it may need or be required to notify any individual(s) as a result of a Security Incident, the Trust shall have the right to control all such notifications and the Transfer Agent shall bear all direct costs associated with the notification, to the extent the notification and corresponding actions are required by U.S. law, and subject to the limitation of liability set forth in the Agreement. Without limiting the foregoing, unless otherwise required by U.S. law, no such notifications shall be made by the Transfer Agent without the Trust’s prior written consent and the Trust shall, together with the Transfer Agent, determine the content and delivery of all such notifications. For the avoidance of doubt, the Transfer Agent shall be solely responsible for all costs and expenses, subject to the limitations of liability under the Agreement that the Trust and/or the Transfer Agent may incur to the extent that they are attributable to or arise from the Transfer Agent’s breach of its confidentiality obligations under the Agreement.

  • Accident and Incident Investigations An accident, injury, critical injury, or hazardous situation will be investigated in accordance with the CJHSC’s investigation protocol.

  • Security Incident Response Upon becoming aware of a Security Incident, MailChimp shall notify Customer without undue delay and shall provide timely information relating to the Security Incident as it becomes known or as is reasonably requested by Customer.
