Sustainability Plan. The Contractor shall adhere to the measurable goals and milestones outlined in its Sustainability Plan, which it submitted as part of its proposal and is attached hereto as Exhibit 2-D. With the Contracting Officer’s written approval, the Contractor may amend its Sustainability Plan. During the Contract Term, the Contractor may be required to provide a detailed update on the goals and milestones outlined in its Sustainability Plan. The Contractor shall provide such an update to the Airports Authority upon request.
Sustainability Plan. Grantees must develop and submit, within 90 business days after the MHU vehicle is functional and begins to serve individuals, a Sustainability Plan for review by System Agency to ensure the long-term viability of grant outcomes. Deliverable completion is contingent upon final System Agency acceptance of the plan. The plan should document how progress towards project goals will continue beyond the funding cycle. The plan will include at minimum: • The changes to systems, policies, and procedures that will enable progress toward project outcomes to continue beyond the funding cycle. Attachment A • The program activities that will be maintained and/or begin after the contract ends as well as the performing entities responsible for those activities. • How community partners and stakeholders will be engaged and supported to advance progress towards project goals. 30 calendar days prior to MHU vehicle been operational, the following must be submitted for approval: • An Eligibility Policy that outlines the organization’s procedures for determining program eligibility and who is responsible for eligibility screening. The policy must include documentation of household and family composition, date of birth, residency, income; presumptive eligibility; calculation of the applicant’s federal poverty level percentage; and eligibility forms. • An active Prescriptive Authority Agreement (PAA) and policy that outlines the organization’s procedures for ensuring properly executed PAAs are in place for each Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA). Expired PAA are not acceptable. • The organization’s Standing Delegation Order and Policy that outlines the organization’s procedures for developing, annually reviewing, properly storing and training staff on SDOs for unlicensed and licensed personnel (not APRN or PA). • Copies of current, valid copies of Professional Licensure or Certification for medical, nursing and other clinical staff. Applicant must submit current, valid copies of driver’s license for anyone hired to operate the vehicle. • Xxxxx of current certificate of registration, $1 million of general liability car insurance, and vehicle maintenance warranty, if purchased new or certified pre-owned, for any MHU associated with this application.
Sustainability Plan. Grantees shall have a plan to ensure long term financial sustainability of the project and services in case the KEHS Child Care Partnership funds are no longer available.
Sustainability Plan. The primary intention of implementing the SLA approach is to ensure better and improved health outcomes for the Sierra Leonean people. Therefore the MoHS places huge emphasis on ensuring that health gains are sustained. When reviewing the submitted project documents, MoHS will pay attention to the sustainability plan described by the implementing partner. Such a plan could include 8.2 and/or 8.3 below. Strengthening Government/national institutions including capacity building of MoHS personnel at implementation sites will be key to ensuring sustainability and safeguard project gains beyond the lifetime of the project and/or funding availability.
Sustainability Plan. As part of the annual Strategic Action Plan, Vendor shall be responsible for developing and implementing a proactive Sustainability Plan for the Dining Services Program that is fully aligned with the University’s sustainability objectives, and on an ongoing basis, includes recommendations for year over year expansion and enhancements to University’s and Vendor’s sustainability initiatives. Vendor’s Sustainability Plan shall address five key areas:

Northwestern shall deliver a reasonable sustainability plan to the City prior to the first Event Date following the date on which a certificate of occupancy is issued for Ryan Field (the “Sustainability Plan”), which Northwestern shall update and deliver to the City annually thereafter. The Sustainability Plan shall describe Northwestern’s intentions with respect to activities related to environmental sustainability in connection with the operation of Ryan Field, such as, by way of example only, the following topics:
Sustainability Plan. The objective of emPLANT+ is to achieve financial sustainability after the end of the 6-year Erasmus Mundus funding. Therefore, the work on the sustainability of the programme will be started immediately in the preparatory year. Several companies and associations have committed to participate in the sustainability of the program and give their advice. The advisory committee will work to obtain company funding to cover the participation costs of the students from intake 4. A second task force will elaborate a list of eligible non-EU grants that students can apply for to cover their living costs (e.g. scholarships in home country, Embassy Grants, Bourse Eiffel, scholarships of International Organisations (UNO, UNESCO, etc.), etc.). The consortium partners are highly committed to the sustainability of the programme and to the objective of obtaining 8 company grants for two years from intake 4.

Sustainability Plan. The governing documents for the Development will contain the provisions substantially as set forth in Exhibit M, attached. No later than the date of recording of the first final plat containing Buildable Lots within the Property, Developer shall adopt procedures to ensure enforcement of the provisions of Exhibit M and such other sustainability requirements as may from time be put in place (“Sustainability Requirements”) by the Developer or the Steamboat 700 master homeowners association (”Master HOA”), such procedures to be submitted to the Director of Planning and Community Development for approval in advance of being adopted. The adopted procedures shall include the requirement that a third party contactor or employee of the Master HOA or the Developer monitor and require enforcement of the Sustainability Requirements on an ongoing basis and that periodic reports be provided to the City at least annually describing the implementation and enforcement of the Sustainability Requirements.
