Supportive Housing. The Project shall include a minimum of 72 Supportive Housing units.
Supportive Housing. The CONTRACTOR shall have a full-time Supportive Housing Specialist dedicated to this Agreement to work with Members to assess housing needs and identify appropriate resources in order to help them attain and maintain housing. The Supportive Housing Specialist shall serve as the internal resource to provide training and technical assistance to the CONTRACTOR’s care coordinators. Supportive Housing specifically targets the following populations: Individuals who are chronically homeless, as defined by the U.S. Department of Housing and Urban Development (HUD) or Precariously Housed; Individuals with frequent or lengthy institutional care; Individuals with Serious Mental Illness (SMI) or chronic SUD, high emergency room or inpatient utilization, with crisis stabilization needs; Individuals with frequent or lengthy adult residential care or treatment stays; Individuals receiving LTSS and experiencing frequent turnover of in-home Caregivers or Providers; and Individuals at highest levels of risk for expensive care and negative outcomes, defined by a Predictive Risk Intelligence System (PRISM) risk score of one point five (1.5) or higher, or defined in another manner as determined by HSD. The Contractor’s identified Supportive Housing Specialist shall be located within one (1) of four (4) regions within the State, and shall be knowledgeable about housing resources in urban, rural, and frontier areas within their specified region. Staff Training The CONTRACTOR shall provide regular and ongoing comprehensive training, including focus on supporting a Trauma responsive lens throughout the system of care for CONTRACTOR staff to ensure that they understand the goals of Turquoise Care, including the integration of Physical Health, LTC, and Behavioral Health, the provisions and limitations of the ABP, and the requirements of this Agreement. As issues are identified by the CONTRACTOR and/or HSD, the CONTRACTOR shall provide timely and targeted training to staff. The CONTRACTOR shall provide an initial orientation and training as well as ongoing training, including training targeted to different types of staff, to ensure compliance with this Agreement. Targeted training must include: Care Coordination; NF LOC Determinations; Setting of Care (SOC) Submissions; Community Benefit Services and Supplemental Questionnaire; Behavioral Health Services; Trauma-responsive training as approved by HSD; Cultural Competency; The CONTRACTOR shall provide training to all staff within the...
Supportive Housing. 1. The SE shall participate in Collaborative supportive housing policy and program development, capacity building and other planning activities to assure effective implementation of supportive housing as requested by the Collaborative and articulated in the Collaborative’s Long Range Housing Plan and/or other Collaborative policy documents. The SE shall identify an individual to serve as the primary, statewide contact with authority to act on behalf of the SE in these activities.
Supportive Housing. Participants will agree to live with other participants and a live-in Arms of Love Coach. Participants will be required to participate in regular house community nights. Each participant will have weekly household responsibilities. Participants are expected to communicate with roommates, treat all housemates with respect as well as comply with requests of the AOL Coach.
Supportive Housing. Funds allow grantees and supportive housing developers/operators offering services to their residents, to utilize the grant for leasing activities, utilities, maintenance, repairs, supportive services, and overall operations of supportive housing sites.
