Supporting Evidence. 5.4.4 The employer may require an employee to provide evidence in relation to a period of xxxxx’s leave where the employee has taken two single days within a calendar year. The employee must then provide a medical certificate issued by a registered health practitioner. The medical certificate must state that in the opinion of the registered health practitioner the member of the employee’s immediate family or household has had or will have a personal illness or injury during the period.
Supporting Evidence. The subsection must include a discussion of the evidence base for including each strategy with respect to the project and the target population and how the strategies will address the priority topic(s) problem(s) or issue(s) noted in the Strategic Approach and Project Design section of the Project Narrative. Applicants should use this section to support the cooperative agreement category for which they are applying, whether causal or feasibility. There are two ways to qualify for the causal category of funding. Causal studies must either build on existing highly rated causal research or be designed with a methodology that could produce highly rated causal research. Well-designed studies that do not meet the threshold for high rated causality methodology would be eligible for feasibility category of funding.

In the absence of such proof, the period of absence from duty shall be without pay or, at the request of the employee, shall be deducted from the Annual leave credits of the employee.
