Summary Evaluation Report. No later than thirty (30) calendar days prior to the end of the school year, the Summary Evaluation Report shall be completed and given to the evaluatee. The summary evaluation conference shall take place as soon as possible after distribution of the Summary Evaluation Report.

Summary Evaluation Report. Within ten (10) school days after the observation(s), the evaluator shall prepare the completed SER and present it for discussion with the unit member.
Summary Evaluation Report. The summary evaluation report will be prepared by the supervisor representing all members of the team and will contain an overall performance rating. In the first, second, and fourth year, the report also will contain a recommendation for renewal or nonrenewal. The report shall consider the worksite observations, student appraisals, evaluators’ reports, evaluatee reports and materials, and other information concerning the performance of core duties, professional competence, professional conduct, professional growth, or college-related service activities of the evaluatee. When the team is examining the development and assessment of course and program-level SLOs, it will not use student performance as a substitute for teacher performance. If the administrator and peer(s) disagree, each member of the evaluation team may include a separate statement addressing the area(s) of disagreement.

  • Evaluation Report 20.5.4.1 The summary evaluation report shall be prepared by the Faculty Evaluation Committee and administrative evaluator(s) and shall include each evaluator’s individual rating. An evaluation report shall not be changed after an employee has signed it, without the knowledge of the employee, and any such changes shall be subject to the grievance procedure.

  • Preliminary Evaluation Site evaluation and planning functions should proceed concurrently with the accomplishment of Schematic Design, Design Development, and Construction Documents. The Design Professional shall conduct a preliminary review of the site based on information furnished by the Owner. The Design Professional will advise the Owner of potential site-related problems that the Design Professional notes from such review.

  • COST REPORT 17 A. CONTRACTOR shall submit separate Cost Reports for Period One, Period Two and Period 18 Three, or for a portion thereof, to COUNTY no later than sixty (60) calendar days following termination 19 of this Agreement. CONTRACTOR shall prepare the Cost Report in accordance with all applicable 20 federal, state and COUNTY requirements, GAAP and the Special Provisions Paragraph of this 21 Agreement.   • Program Evaluation The School District and the College will develop a plan for the evaluation of the Dual Credit program to be completed each year. The evaluation will include, but is not limited to, disaggregated attendance and retention rates, GPA of high-school-credit-only courses and college courses, satisfactory progress in college courses, state assessment results, SAT/ACT, as applicable, TSIA readiness by grade level, and adequate progress toward the college-readiness of the students in the program. The School District commits to collecting longitudinal data as specified by the College, and making data and performance outcomes available to the College upon request. HB 1638 and SACSCOC require the collection of data points to be longitudinally captured by the School District, in collaboration with the College, will include, at minimum: student enrollment, GPA, retention, persistence, completion, transfer and scholarships. School District will provide parent contact and demographic information to the College upon request for targeted marketing of degree completion or workforce development information to parents of Students. School District agrees to obtain valid FERPA releases drafted to support the supply of such data if deemed required by counsel to either School District or the College. The College conducts and reports regular and ongoing evaluations of the Dual Credit program effectiveness and uses the results for continuous improvement.

  • Condition Report The landlord and tenant are required to inspect the residential unit together at the beginning and end of the tenancy and complete a written condition report. If the landlord allows the tenant to have a pet after the start of the tenancy, an inspection report must be done on the day the tenant starts keeping a pet or on another day mutually agreed to by the landlord and tenant, unless the tenancy started on or after January 1, 2004, and a condition inspection report was completed at that time. A report may describe any damage, how clean each room is, and the general condition of the residential unit including: the floors, carpets, appliances, and paint on the walls. The report must be signed and dated by both the landlord and the tenant who made the inspection, and each should keep a copy. Change of Landlord – A new landlord has the same rights and duties as the previous one and must follow all the terms of this agreement unless the tenant and new landlord agree to other terms.

  • Project Monitoring Reporting Evaluation 1. The Project Implementing Entity shall monitor and evaluate the progress of the Project and prepare Project Reports in accordance with the provisions of Section 4.08(b) of the General Conditions and on the basis of indicators agreed with the Association. Each such Project Report shall cover the period of one calendar semester, and shall be furnished to the Recipient not later than eight (8) weeks after the end of the period covered by such report for incorporation and forwarding by the Recipient to the Association of the overall Project Report.

  • Evaluation Schedule CLIENT REFERENCES The Tenderer shall provide details of his performance on each of the previous projects listed in the “Relevant Experience” returnable schedule. “Client Reference Scorecards” will be completed by each of the respective Clients for the projects listed in the “Relevant Experience” returnable schedule. The following are to be completed by the Client and Principal Agent and is to be supported in each case by a letter of award and the works completion certificate. Both Client and Principal Agent must sign and stamp the documents, failure to obtain both signatures and stamps will result in no allocation of points.
