Submission of Annual Representations and Certifications. The CNA shall review the Representations and Certifications submitted by NPAs to identify errors, omissions, or anomalies as compared to information available to the CNA, such as prior submissions or regulatory review and assistance visit reports, or guidance from the Commission, and resolve them with the NPA within thirty (30) days of submission. The CNA shall, as part of the terms of the written agreement with the NPAs, include a provision in which the NPA agrees to provide accurate, timely, and ongoing submissions for the Representations & Certifications form. The CNA shall forward to the Commission, no later than December 1, annually, a completed original and legible copy of the appropriate Annual Certification for the fiscal year ending the preceding September 30 for each of the NPAs, data extract containing all information within the annual Representations & Certifications and a summary report that contains the following information:

The CNA will use good faith efforts to review for irregularities and outliers, such as discrepancies between hours, and labor, or other anomalous data when compared to previous quarters or years of data, and resolve them with the NPA within the first thirty (30) days of submission. (See Attachment 6 for AR&C Review Process.) The CNA shall forward to the Commission, annually, by December 1, electronic copies of the appropriate AR&C for the fiscal year ending the preceding September 30 for each of the NPAs, and data extract containing all within the AR&C and a summary report that contains the following information:

Submissions serve as an official certification that the CNA has validated data provided to the Commission. The CNA must perform its due diligence to ensure all information received from the NPAs, is validated prior to the CNA submitting it to the Commission. The CNA shall forward to the Commission, no later than December 1, annually, a completed legible original copy of the appropriate Annual Certification for the fiscal year ending the preceding September 30 for each of the NPAs, and data extract containing all information within the annual Representations & Certifications and a summary report that contains the following information:
