Student Questionnaires. Student questionnaires will be distributed to students and collected by faculty supervisors for purposes of gaining student input regarding any and all aspects of a course including classroom facilities, scheduling, textbooks and other materials, equipment, method and style of presentation, and methods of student evaluation. Administration will share aggregate results of the questionnaires with faculty at the end of the course for purposes of modifying or improving the course. These questionnaires will not be used as part of the formal faculty evaluation process and, therefore, would not be used as a basis for discipline, promotion, retention, salary increments, granting or denial of appointments, or other adverse or positive action involving faculty members. However, student questionnaires may prompt additional faculty observations by the faculty’s direct supervisor. Such additional observations shall not take place without prior arrangement with the faculty member to be observed.

Student Questionnaires. Student questionnaires shall be administered every quarter (excluding summer) and for every class taught. They will be delivered utilizing automated student questionnaires via EvaluationKit or appropriate technology. Certain programs may use a different student evaluation tool agreed upon by both parties (see Appendix) If the faculty member disagrees with any student comments, the faculty member shall have the right to place their disagreement in writing and submit it to the Vice President for Instruction. Outside of evaluation years, faculty members shall have the right to opt-out of student questionnaires for the academic year by submitting a request to the Director of eLearning during the first 30 calendar days of fall quarter or the first 30 calendar days of the next quarter taught if not teaching fall quarter. Opt-out requests must be renewed annually.
Student Questionnaires a. Student questionnaires are set forth as a means to provide information about the teaching and learning process and as feedback to help improve effectiveness with which faculty members carry out their responsibilities. They will be used for the improvement of course content and presentation, for the enhancement of teaching and learning, and as one among various factors for personnel decisions such as continuance, tenure, and promotion. By themselves, student questionnaires shall not be used to initiate disciplinary action against a faculty member.
