The CONSULTANT shall acquire and maintain statutory workmen's compensation insurance coverage, employer's liability insurance coverage in the amount of $1,000,000.00, and comprehensive general liability insurance coverage and professional liability insurance in the coverage amount of $1,000,000.00 and $1,000,000.00 respectively, and shall provide certificates of insurance to the OWNER. Said insurance shall provide OWNER thirty (30) days notice prior to cancellation. Exhibit A - Scope of Services The original agreement with HNTB provided for splitting out and updating the original Monticello Road (Shawnee Mission Parkway to 7900 Block), dated February 9, 2009, to two separate projects (Phase 1 and Phase 2), and then advancing the design to 95% Final Design plans for each phase and Bidding Plans for Phase 1. Supplemental Agreement No. 1 added additional design services, construction administration (Phase 1 only), and construction observation services (Phase 1 only). Supplemental Agreement No. 2 added supplemental design services and survey related services for Phase 2 only. Supplemental Agreement No. 3 added supplemental construction observation and administration services for Phase 1 related to the extension of the construction schedule. This Supplemental Agreement No. 4 with HNTB is for supplemental design services, construction engineering and construction observation services for Phase 2 only. The following are the anticipated services, deliverables, schedule, and assumptions for the scope of work.
