Sick Leave Notification. An employee who is not on approved leave and unable to report to work shall contact their immediate supervisor, and the Communications Center to notify them they are unable to report for work. This notification must be made at least one (1) hour prior to their scheduled shift. If the employee is off for more than one (1) day they will notify their supervisor and the Communications Center each day sick leave is requested unless prior notification was given.
Sick Leave Notification. An employee shall make every effort to notify a supervisor at least one (1) hour prior to his/her scheduled work shift when ill.

Sick Leave Notification a. Employees have the responsibility to assure that the supervisor is notified of their need for unplanned sick leave. The employee will make the notification personally, unless the degree of injury or illness prevents it.

Employees will take all reasonable steps possible to notify their defined Supervisor of their impending absence, prior to the start of the work day or as soon as is known, where a health situation will necessitate an anticipated extended absence. For the purposes of Article 21, telephone conversations, voice mail, and/or e-mail will be accepted forms of notification.
Sick Leave Notification. Nurses shall notify the Employer at least one and one-half (1-1/2) hours in advance of the nurse’s scheduled shift if the nurse is unable to report for duty as scheduled. ADDENDUM 4 PROVIDENCE ST. XXXXX HOSPITAL 7/70 SHIFT SCHEDULE In accordance with Section 7.3 of the Agreement between the Hospital and the Union, nurses may, on an individual basis, agree to work seven (7) consecutive ten (10) hours shifts followed by seven (7) consecutive days off with the consent of the Employer. All existing contractual provisions shall apply unless otherwise provided for herein.
